Wegovy has two doors most people do not know about, and both change what you pay. One is a heart indication that some plans treat differently from weight. The other is a tablet that costs less than the pen. The steps are the same as for any prescription drug: qualify, get it written, get it paid for. The doors are where Wegovy differs from everything else on this site.
1. Do you qualify
Two indications, and they are worth reading separately.1
Weight. "To reduce excess body weight and maintain weight reduction long term in adults with obesity, or in adults with overweight in the presence of at least one weight-related comorbid condition."1 The trials behind it used a BMI of 30, or 27 with a comorbidity,2 and that is what prescribers and plans apply.
Heart. "To reduce the risk of major adverse cardiovascular (CV) events (CV death, non-fatal myocardial infarction, or non-fatal stroke) in adults with established CV disease and either obesity or overweight."1 This is the door Zepbound does not have. If you have had a heart attack, a stroke or have diagnosed cardiovascular disease, Wegovy can be prescribed for that, and a plan that excludes "weight-loss drugs" may cover it under the cardiovascular indication, because the prescription is not for weight.
This is the single most useful thing on this page for anyone with heart disease and an insurer that says no to obesity drugs. Ask the prescriber which indication the prescription is written under, and ask the plan whether it covers Wegovy for cardiovascular risk reduction. Those are different questions with, sometimes, different answers.
Wegovy is not for type 2 diabetes. Ozempic is the same molecule in the diabetes box, and the box decides your coverage.
2. Who writes it, and in what form
Any licensed prescriber, in person or by telehealth, with no specialist requirement. The choice you make at this step is the form.
The pen is a weekly injection, climbing from 0.25 mg through 0.5, 1, 1.7 to 2.4 mg, with a 7.2 mg HD pen above that for people who plateau; Wegovy 7.2 mg has the trial and the price.
The tablet is a daily pill, "the only semaglutide tablet that's FDA-approved for weight loss in adults",4 climbing to a 25 mg maintenance dose.1 If needles are the reason you have not started, this is the reason to. It is also cheaper.
The three prescribing routes are the same as for any GLP-1: your own doctor (cheapest with a plan, and the one that will file prior authorisation), a telehealth program (fastest; Ro, Sesame and others dispense branded Wegovy with a membership, priced in what each program charges), or Novo's own NovoCare, which pairs a consultation with its self-pay pharmacy.
3. How it gets paid for
Route A: a commercial plan that covers it
Novo's savings card brings a covered prescription to as low as $25 a month for commercially insured patients, government plans excluded. Prior authorisation is close to universal. Whether your plan covers Wegovy for weight is plan-specific; here are eight insurers in their own words:

Route B: refused
A denied prior authorisation on a plan that lists Wegovy is appealable, and the cardiovascular indication is the strongest ground if it applies to you. A plan-level exclusion is not appealable through the insurer. Which no you got decides the next step.
Route C: pay Novo directly

Read the intro price for what it is. The $199 is "for 2 monthly fills" at the two starting doses, "then $349 per month".3 It is a real discount on the first two months and it is not the price of Wegovy. Budget for $349 for the pen, or $149 to $299 for the tablet depending on where your dose settles.
Route D: compounded semaglutide
Wegovy's molecule is sold compounded by licensed pharmacies through telehealth programs, at a fraction of Novo's price and without FDA approval of the finished product. The trade is real and it is set out in is compounded semaglutide safe. Today's verified prices:
4. The first month
The pen starts at 0.25 mg for four weeks and climbs every four weeks. The tablet has its own escalation on the label. Either way the gut effects arrive with dose steps and ease within them, and the labels' one instruction is to climb slowly; the numbers are in the side-effect guide.
The honest summary
Qualifying is a BMI of 30, or 27 with a condition, or established heart disease. Any prescriber can write it, as a pen or a pill. Paying is where the routes split: $25 with coverage, $149 for the tablet or $349 for the pen from Novo, or $69 and up for the same molecule without FDA review. Two things to ask before you book: whether your plan covers Wegovy, and if you have heart disease, whether it covers it for that.
