Titration schedule: common questions
What is the Zepbound titration schedule?
On the Zepbound label: 2.5 mg once weekly for 4 weeks, then 5 mg, and the dose may rise in 2.5 mg steps after at least 4 weeks on each dose. The maintenance dose is 5, 10 or 15 mg, and 15 mg is the maximum.
What is the Wegovy titration schedule?
On the Wegovy label: 0.25 mg weekly for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16, then a maintenance dose of 1.7 or 2.4 mg (2.4 mg recommended).
How much does the first year of Zepbound cost without insurance?
At Lilly's self-pay prices on the fastest label schedule to 15 mg: 2.5 mg $299, 5 mg $399, 7.5 mg $449, 10 mg $449, then $449 a month, $5,188 for the first 12 fills. Prices read Oct 3.
How much does the first year of Wegovy cost without insurance?
At NovoCare Pharmacy's self-pay prices: $199 for each of a new patient's first two fills, then $349 a month at every dose from 0.25 to 2.4 mg, $3,888 for the first 12 fills. Prices read Oct 3.
Does a slower titration cost less?
On Zepbound, yes: Lilly's self-pay price rises with the dose, so every extra month on a lower dose is cheaper. On Wegovy, no: after the first two fills NovoCare charges the same price at every dose from 0.25 to 2.4 mg. The labels set four weeks as the minimum time on each dose; your prescriber sets the pace.
Why is Mounjaro's year total not shown?
Lilly publishes Mounjaro's self-pay price only as "starting at $499/month" and no price for the higher doses, so a year total would be a guess. Mounjaro is approved for type 2 diabetes, not for weight loss; Zepbound is the same molecule approved for weight loss.
