Which state Medicaid programs cover GLP-1s for weight loss?

13, as of January 2026 (KFF): Delaware, Kansas, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, North Carolina, Rhode Island, Tennessee, Utah, Virginia and Wisconsin.

California, New Hampshire, Pennsylvania and South Carolina covered GLP-1s for obesity in October 2025 and had ended it by January 2026. North Carolina dropped GLP-1 coverage in October 2025 over a budget stalemate and reinstated it in December 2025. Every other state covers GLP-1s only for the uses Medicaid must cover: type 2 diabetes, plus Wegovy for heart risk and Zepbound for sleep apnea. Does Medicaid coverage change what gets prescribed?

In the 13 states KFF lists as covering GLP-1s for weight loss, the brands approved for weight management (Wegovy, Zepbound and Saxenda) were a median 52% of Medicaid's GLP-1 prescriptions in 2025. In the other 38, 2.3%.

Where weight loss is not covered, Medicaid’s GLP-1s are diabetes drugs: Ozempic, Trulicity and Mounjaro. The 2025 prescriptions predate the January 2026 snapshot, so states that ended coverage late in 2025 still show high shares. KFF reports that California, New Hampshire, Pennsylvania and South Carolina ended coverage of GLP-1s for obesity between October 2025 and January 2026. North Carolina dropped coverage in October 2025 and reinstated it in December 2025.

Does Medicaid cover GLP-1s for diabetes in every state?

Yes. Coverage for weight loss is optional for each state, but Medicaid must cover GLP-1s approved for type 2 diabetes, and the required uses also include Wegovy for heart risk and Zepbound for sleep apnea (KFF). Prior authorization is typical.

Has my state joined the BALANCE model?

We could not find a published list of participating state Medicaid programs. States could join on a rolling basis from May 1, 2026 through January 1, 2027, with a July 31, 2026 application deadline (KFF). Ask your Medicaid plan.
