State employee plans as quoted from each plan's own document; Medicaid from KFF's January 2026 map; cheapest all-in monthly price among partner programs that list the state, checked weekly. All 51 rows, with every column, are in the CSV and JSON.
How the number is computed
Each state employee plan's stance is read from the plan's own published document (benefit guide, plan announcement, formulary or board minutes), quoted verbatim with the day we read it. "Covers with limits" means the plan pays for weight-loss GLP-1s only under a stated condition. Counts are of the plans we have verified; a state we have not read is counted as not yet verified, never guessed.
Included
- State employee health plans for active, non-Medicare members.
- GLP-1s prescribed for weight loss (Wegovy, Zepbound, Saxenda and the plan's own named drugs).
- Medicaid's weight-loss answer under fee-for-service, as KFF mapped it in January 2026.
Excluded
- Coverage for type 2 diabetes, heart risk or sleep apnea, which most plans keep.
- Private employers and individual-market plans, except the insurer policies quoted on each state page.
- Compounded GLP-1s, which are not FDA-approved.
What this does not show
- Plans change at renewal, often January 1 or July 1; each row carries the day we read its source.
- A plan's limits are summarized from its own wording; the full quote is on the state page and in the CSV.
- KFF's Medicaid map is dated January 2026 and does not include later state decisions.
