What compounded tirzepatide does, beyond the scale

See every price

The most weight lost, head to head

In SURMOUNT-5, tirzepatide produced a 20.2% mean body-weight reduction at 72 weeks against semaglutide's 13.7%, across 751 adults. That is the molecule, measured as the brand.*

The cheapest route to it

Branded tirzepatide starts far higher than the compounded prices on this page. The board below is every verified price we found today, ranked on the all-in monthly cost.

The evidence is borrowed

Every trial figure here was produced with Lilly's product. No compounded tirzepatide has been trialled, so the numbers describe the molecule, not the copy.*

Is compounded tirzepatide the right price for you?

Same appetite pathway, very different needles, prices, and evidence. Every column links to its live prices.

Worth knowing before you buy: Compounded semaglutide starts at $69/mo, $16 less than compounded tirzepatide on the same appetite pathway. Different medicine, different evidence, real saving.

Insurance

Never covered

Insurers do not reimburse compounded drugs, and manufacturer savings cards apply only to the branded product. Every price on this page is cash.

What you actually pay

Cash, all in

The programs below bundle the prescribing visit into the monthly price. Where a program charges a separate membership fee, we add it in and rank on the total, because that is what leaves your account.

If the category closes

Zepbound is the fallback

Compounded supply depends on regulatory posture that has already tightened twice. Branded tirzepatide is the route that survives an enforcement change, at a materially higher price.

No insurer has published a compounded tirzepatide position we could verify yet; we recheck weekly and post verdicts on the coverage fact sheets the moment one appears.

The same molecule, three ways to buy it. Manufacturer prices are its own published self-pay list and carry no link; program prices are all in, membership included, read off each seller’s own page.

Compounded tirzepatide at Collective is $213 a month under Zepbound’s starting self-pay price, $2,561 over a year at those prices. Zepbound is FDA-approved; compounded tirzepatide is not.

How to choose a compounded tirzepatide program

How to choose a compounded tirzepatide program: 7 checks

Price ranks the list. These decide whether the cheapest is cheapest for you. Each count is from the programs on this page, today.

The price at your dose
2 of 2 hold one price at every dose
Advertised prices are usually the starting dose. A seller that charges more as you step up can end up dearer than one with a higher entry price.
How long you commit
14 of 14 sell compounded tirzepatide month to month
13 of 14 publish cancel-anytime terms. A low price that needs a 6 or 12-month prepay is a loan to the seller if you stop early.
Fees on top
1 of 14 add a membership fee
None charge a separate consultation. We rank on the all-in price so a fee cannot hide.
Insurance
2 of 14 take insurance
Most cash programs cannot bill your plan. If your plan covers a brand GLP-1, a copay can beat every cash price here.
Brand or compounded
5 of 14 also sell an FDA-approved brand
Compounded versions are cheaper and not FDA-approved. If you want the approved drug later, a seller that stocks both saves a switch.
Who stands behind it
1 of 14 carry a LegitScript certificate we confirmed
A real program names its clinician network and its pharmacy. LegitScript certifies telehealth sellers; check a certificate on legitscript.com, not on a badge the seller hosts.
Where you live
5 of 14 ship to all 50 states
State rules decide who can prescribe and dispense. Check your state before you pay an intake fee.

Red flags: walk away if

  • The box arrives warm or with too little ice. FDA says do not use it.[2]
  • The seller will not name the pharmacy, or the one on the label is not on your state board's register.[2]
  • Nobody assesses you before the sale. Compounded drugs are for patients a clinician has assessed.[2]
  • Your dose comes in mg with no syringe units, or above the dose on the brand's label.[2] Convert mg to units
  • You are told to keep using a vial more than 28 days after first opening it.[2]

Compounded tirzepatide price: common questions

How much does compounded tirzepatide cost without insurance?

From $85 a month all in at Collective, the cheapest of 14 programs we track for it, checked October 2026.

Where is the cheapest place to get compounded tirzepatide?

Collective, at $85 a month all in month to month. We compare 14 programs on their own published prices, membership fees included, and re-check every week.

Will insurance cover compounded tirzepatide?

It depends on your plan, not the drug: many plans cover GLP-1s for type 2 diabetes and exclude them for weight loss, and coverage changes by employer and year. Our coverage pages quote what each major insurer publishes; if your plan says no, the cash prices on this page are the fallback.

Can I pay for compounded tirzepatide with an HSA or FSA?

Usually, for the medication. The IRS counts "prescribed medicines and drugs" as medical expenses, and an HSA pays for "medical care" as defined in Code section 213(d) (IRS Publications 502 and 969, read October 2026). Program fees count when the weight loss is "a treatment for a specific disease diagnosed by a physician (such as obesity...)". Your plan administrator decides what it reimburses, so check before you pay; we track prices, not tax rules.

Do I need a prescription for compounded tirzepatide?

Yes. Every GLP-1 here is sold on a prescription from a licensed clinician after an intake; 14 of the 14 programs we track say who does the prescribing, and none charges a separate consultation fee. FDA says compounded drugs "should only be used in patients whose medical needs cannot be met by an FDA-approved drug", so a seller that ships without assessing you is a red flag.

Can I change my compounded tirzepatide dose, and does it cost more?

Your prescriber sets the dose. On price, 2 of the 2 sellers that publish dose-level prices charge the same at every dose and 0 charge more as you step up. FDA has received reports of dosing errors with compounded injections, some needing hospital care, so convert mg to syringe units from your own vial's label (our dose converter does the arithmetic).

Is compounded tirzepatide still legal?

It depends on who compounds it and why. FDA says "tirzepatide and semaglutide do not currently appear on the 503B bulks list or on FDA's drug shortage list", so the shortage that allowed mass copying is over (read October 2026). FDA has also proposed excluding both from the 503B bulks list. State-licensed pharmacies can still compound for an individual patient's documented need. Ask any seller which pharmacy fills your order and look it up with your state board.
