LillyDirect vs NovoCare Pharmacy: every self-pay price by dose, the refill rules, and who can use them

LillyDirect sells Zepbound vials and KwikPens from $299 a month at 2.5 mg, $399 at 5 mg and $449 at every higher dose, but only if each refill is bought within 45 days of the last delivery; miss it and 10 to 15 mg costs $699. NovoCare Pharmacy sells every Wegovy pen dose for $349 ($199 for a new patient's first two boxes), Wegovy HD for $399, and the Wegovy pill from $149. Both are cash only, and neither counts toward your deductible.

Key takeaways

  • 1LillyDirect: Zepbound vial or KwikPen $299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg) when each refill lands within 45 days of the last delivery. Late refills cost $499 at 7.5 mg and $699 at 10 to 15 mg.
  • 2NovoCare Pharmacy: Wegovy pen $349 at every dose from 0.25 to 2.4 mg, $199 for a new patient's first two boxes through 31 December 2026, Wegovy HD 7.2 mg $399, Wegovy pill $149 to $299, Ozempic pen $349 ($499 at 2 mg).
  • 3Both are cash only. Neither bills insurance, and the money does not count toward a deductible. NovoCare says government-insured patients may self-pay; Lilly's card terms exclude government beneficiaries.
  • 4Medicare's GLP-1 Bridge charges $50 a month for Wegovy (pen or pill), Foundayo and the Zepbound KwikPen, but not Zepbound vials, from 1 July 2026.
  • 5Compounded tirzepatide on our board starts at $85 a month all in and compounded semaglutide at $69, but compounded drugs are not FDA-approved.

LillyDirect is cheaper at the bottom of the ladder and NovoCare Pharmacy is cheaper at the top, and the real difference is the rules. Lilly sells Zepbound, as a vial or a KwikPen, for $299 a month at 2.5 mg, $399 at 5 mg and $449 at every dose from 7.5 to 15 mg, as long as you buy each refill within 45 days of your last delivery.23 Novo's NovoCare Pharmacy sells every Wegovy pen dose from 0.25 to 2.4 mg for $349, Wegovy HD 7.2 mg for $399, and gives a new patient the first two boxes at $199.15 Miss Lilly's window at 10 mg or above and that month costs $699.3

45 days

Lilly's refill window. Buy your next Zepbound vial or KwikPen within 45 days of the date the last one was delivered and the 7.5 to 15 mg doses stay at $449; buy it later and you pay the regular price, up to $699.3 NovoCare has no equivalent window.

Both are the manufacturers' own cash counters. Neither one bills insurance, neither one writes prescriptions, and the money you spend at either does not count toward a deductible.123 What follows is every price both publish, dose by dose and month by month, a dated history of how those prices fell, who is allowed to use each pharmacy, the telehealth services each one points to, how to order step by step, and how the two compare with the cheapest programs on our board, which you can see live below.

Prices here were read at the makers' own pages and releases on 15 October 2026. The live slots (rankings, charts and ladders) update on their own.

Two things that table makes plain. First, these are not rival stores for the same product: Lilly only sells Lilly drugs and Novo only sells Novo drugs, so the real choice is between Zepbound and Wegovy (or between Foundayo and the Wegovy pill), with the pharmacy following from it. Second, the cheaper channel flips depending on the dose. If you are early on the ladder, Zepbound's $299 and $399 sit close to Wegovy's $349. Further up, Lilly's $449 sits above Novo's $349 even when you keep to the window, and the gap widens if you do not.

LillyDirect Zepbound prices by dose: vial and KwikPen

Since 23 February 2026, Lilly prices the Zepbound vial and the new four-dose KwikPen the same on LillyDirect: "all doses in either multi-dose KwikPen or single-dose vial at the same self-pay price, starting at $299".1 One month is one KwikPen or four vials, a 28-day supply.3

12.5 mg$449$699

Per 28-day month, self-pay, read at zepbound.lilly.com and in Lilly's December 2025 release on 15 October 2026.23 Taxes and fees may apply.

Three details the headline price leaves out.

Supplies cost extra. The KwikPen needs pen needles, which Lilly says "are required and sold separately".3 The vial needs syringes and needles, which LillyDirect sells alongside the vials.5 Budget a few dollars a month on top of the drug.

The single-dose pen is a different product. The autoinjector most people picture when they hear "Zepbound" is not on this price list. Lilly's own wording: "The $299 per month is for the Zepbound KwikPen or vial, not Zepbound single-dose Pen."3 The single-dose pen is the version your insurance or Lilly's commercial savings card is built around; for that route read the Zepbound savings card.

Retail pharmacies match it for the KwikPen only. Since 16 March 2026, Lilly's KwikPen Self-Pay Savings Card gives you LillyDirect's self-pay price at major pharmacies too, but "the card applies only to the Zepbound KwikPen and does not apply to single-dose vials or the single-dose pen".4 Vials stay a LillyDirect product, with Walmart as the one pickup partner.8

For the whole Zepbound cash picture, including what programs charge to bundle the prescription, see Zepbound cost without insurance.

Lilly's 45-day refill rule, worked through

This is the rule that most often surprises people on LillyDirect, and the one that moves the price the most. Lilly calls it the Zepbound Self Pay Journey Program. You do not sign up: you "automatically receive the $449 purchase offer on your first purchase" of 7.5 mg or higher, and you keep it only if each refill is completed "within 45 days of the delivery/received date of your previous" order.3

Read that carefully, because the clock is not where most people assume it is.

  • The clock starts at delivery, not at your last dose. A month of Zepbound is 28 days. If your box arrives on 1 March and you start it that day, you run out around 29 March, and your window closes on 15 April. That leaves about two weeks of slack for a delayed refill, a pause for side effects, or a late prescription renewal.
  • The rule covers 7.5 mg and above only. At 2.5 mg and 5 mg the regular price and the offer price are the same ($299 and $399), so a late refill at those doses costs nothing extra.3
  • The penalty depends on the dose. A late 7.5 mg month costs $499 instead of $449. A late 10, 12.5 or 15 mg month costs $699 instead of $449.3
  • It applies to the KwikPen and to vials. Lilly's vial releases carry the same rule: the program "lowers the price of the 7.5 mg, 10 mg, 12.5 mg and 15 mg vials only on first fill and when prescriptions are refilled within 45 days of their previous delivery".2

Three ways people keep the lower price: set a calendar reminder for day 30 after each delivery, not day 28 after the first dose; ask your prescriber to renew before the last refill on the prescription runs out, since an expired prescription is the commonest reason a refill slips; and if you are pausing on purpose, know that restarting more than 45 days after the last delivery resets you to the regular price until the next on-time refill.

NovoCare has no window like this. A Wegovy box costs the same whether you reorder after 28 days or after 60.13

Foundayo at LillyDirect: the pill

Foundayo (orforglipron) is Lilly's once-daily GLP-1 pill for weight loss, available since 9 April 2026 through LillyDirect, telehealth providers and retail pharmacies. Self-pay starts at $149 a month at the lowest dose, and a Foundayo month is 30 days.11 When Lilly announced the price plan in November 2025, it described the lowest dose at $149 "with additional doses up to $399".9 The current ladder, with Lilly's temporary reductions and its 45-day purchase offer on the two highest doses, is below.

How Foundayo compares with the Wegovy pill for the same money is its own question; Foundayo vs the Wegovy pill covers it, and our Foundayo guide covers the drug.

NovoCare Pharmacy Wegovy prices by dose: pen, HD and pill

NovoCare publishes a full price table in its pharmacy terms, including what two and three boxes cost if you order a longer supply. A Wegovy month is one box of four pens, a 28-day supply; a Wegovy pill month is one bottle of 30 tablets.13

Who counts as a new patient. The $199 price is for someone who has "never filled a Wegovy injection pre-scription through Novocare or Novocare Legacy" (the typo is Novo's).13 It covers at most the first two 28-day fills, at 0.25 or 0.5 mg, and the prescription has to be written and received by 31 December 2026 and shipped by 31 January 2027.13 The third box is $349 whatever the dose.

Ninety days at a time is allowed. NovoCare says it "offers a 90-day supply option for prescriptions" when your prescriber writes one.16 It saves reorders, not money: three boxes cost exactly three times one.

You do not need a coupon. "You don't need to activate a copay or saving card to get these prices through NovoCare Pharmacy."16 If you would rather collect at a local pharmacy, the same self-pay prices need the Wegovy savings offer at the counter instead: "To pay these prices, you must fill your prescription using NovoCare Pharmacy or use the savings offer."15 Our Wegovy coupon guide walks through the card.

Ozempic at NovoCare Pharmacy: pen and pill

NovoCare also sells Ozempic for cash. Ozempic is approved for type 2 diabetes, not for weight loss, and NovoCare only fills a prescription written "for an FDA-approved indication within that product's labeling".12 An Ozempic month is one pen (28 days) or one bottle of 30 tablets.14

Step by step: which maker charges less at each rung

Each drug climbs its own ladder of strengths, and the strengths are not equivalent between the two drugs, so this is not a dose-for-dose medical comparison. It answers a money question: if you are on the first, second or sixth strength of your drug, what does that month cost from each maker? Your prescriber decides which strength you take and when it changes.

On the makers' own counters, Wegovy is the cheaper drug to buy for cash at every rung but the first, and the difference is largest once you leave the starting doses. That is a price fact, not a recommendation: Lilly cites a head-to-head study in which people on Zepbound lost more weight than people on Wegovy,1 and the drug that works for you is worth more than the monthly gap. The pills follow the same shape at the bottom (Foundayo and the Wegovy pill both start at $149),1115 and the ladder slots above show where each tops out.

How LillyDirect and NovoCare prices changed since 2024

Nobody else keeps this list in one place. Every row is dated and taken from the maker's own release or terms. It shows how quickly the cash price of a branded GLP-1 has fallen: the Zepbound 2.5 mg vial went from $399 to $299 in 15 months, and NovoCare's Wegovy price from $499 to $349 in eight.

One quiet change sits inside that table. NovoCare opened in March 2025 for "uninsured patients or eligible patients with commercial insurance who do not have coverage".19 Its terms now say patients "must either be uninsured or have insurance (either commercial or government-funded), but decide to self-pay".12 If you were turned away in 2025 because you have Medicare, that has changed.

The same trend shows up across the whole cash market. Here is our price index for tirzepatide, every program we track, since we started recording.

Nothing you spend counts toward your plan. NovoCare makes you agree that "You will not attempt to count the cost of this medicine toward your insurance deductible or out-of-pocket limit."12 Lilly's terms say the same of the KwikPen card.3 If you are close to meeting a deductible, paying cash does not get you there.

The Bridge has its own limits. It runs from 1 July 2026 for people with Part D, it covers "Zepbound (KwikPen only" and not "single-dose Zepbound vials or pens", and you are not eligible if you have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease, because your Part D plan may cover the drug for those instead.23 Our Medicare guide has the full criteria.

HSA and FSA money works at NovoCare. It takes "credit cards, Apple Pay, Google Pay, and FSA/HSA cards".16

If you have insurance and want to know whether you are better off using it, GLP-1 cost with insurance walks through the arithmetic.

Telehealth: who writes the prescription

Neither LillyDirect nor NovoCare employs prescribers. Both need a valid prescription from a licensed clinician, and both point you to independent telehealth services if you do not have one.

LillyDirect has offered "access to independent telehealth providers" since launch,10 and Lilly says Foundayo is available "via LillyDirect and telehealth providers".11 Lilly's directory changes, so check the provider list on LillyDirect when you order. Any US clinician can send the prescription: LillyDirect "is listed as a pharmacy option in all major electronic health records (EHR) systems".4 Lilly also runs "a multi-step verification process" so vials go only to patients with "a valid, on-label electronic prescription".5

NovoCare lists telehealth providers on its find-care page, including Hims & Hers, LifeMD, Sesame and WeightWatchers, and adds that "Novo Nordisk operates independently and does not endorse these health care providers or organizations".18 Since 31 March 2026 several of them also sell Wegovy on a subscription that Novo prices below NovoCare's own monthly rate:

Available March 31, 2026, through Ro, WeightWatchers, LifeMD, with Hims & Hers, Sesame, and other telehealth providers coming online soon ... 3-month subscription: $329/month, a savings of $240 per year 6-month subscription: $299/month, a savings of $600 per year 12-month subscription: $249/month, a savings of $1,200 per year

Novo Nordisk, 31 March 2026, Wegovy pen subscription terms22

Those subscription prices are for the drug; the telehealth service's own visit or membership fee sits on top, and the commitment runs for the whole term. The programs on our board quote one all-in monthly number that already includes the clinician, which is the fairest way to compare them with a pharmacy that needs a prescription from somewhere else. Here is the live ranking for branded Zepbound.

And for branded Wegovy, pen or pill:

Shipping, pickup and the pharmacies behind each counter

Both makers ship free, and both now offer a pickup route. The pharmacy that fills your order is not the maker itself.

  • LillyDirect delivery. Orders "ship free of charge", filled by third-party pharmacy services working under LillyDirect.10
  • LillyDirect pickup. Vials can be collected at Walmart pharmacies at the same price as delivery; you choose at checkout.8 The KwikPen can be bought at major retail pharmacies at the self-pay price with Lilly's savings card.4
  • NovoCare delivery. "Yes, shipping and delivery are free."16 Your prescription is taken in by CoAssist Pharmacy, which Novo names as "a dispensing pharmacy for NovoCare Pharmacy",17 and may be filled by CenterWell Pharmacy or a BlinkRx network pharmacy; that network is licensed in all 50 states, the District of Columbia and Puerto Rico.12 Expect texts from those names, not from Novo.
  • NovoCare pickup. NovoCare can transfer the prescription to a local pharmacy for pickup.16
  • Timing. NovoCare says processing "may take a few days" after the prescription arrives, and you schedule delivery once you have paid.16 Lilly does not publish a delivery time in its releases. For the 45-day rule, Lilly counts from the delivery or received date, so track it.3

How to order from LillyDirect, step by step

  1. Get a prescription for the Zepbound vial or KwikPen (not the single-dose pen), or for Foundayo, from your own clinician or a telehealth service.3
  2. Have it sent to LillyDirect. Your prescriber picks LillyDirect in their electronic health record system, where Lilly says it is listed in all the major ones.4
  3. Verify and pay. LillyDirect checks the prescription is valid and on-label before it dispenses.5 You pay the self-pay price for your dose; there is no insurance step.
  4. Choose delivery or pickup. Free home delivery, or Walmart pickup for vials.8 Add syringes and needles for vials, or pen needles for the KwikPen.53
  5. Note the delivery date. From 7.5 mg up, your next order must be completed within 45 days of it to keep the $449 price.3

How to order from NovoCare Pharmacy, step by step

  1. Get a prescription for Wegovy (pen, HD or pill) or Ozempic, written for an approved use, with your email and mobile number on it.1216
  2. Have it sent to NovoCare Pharmacy electronically. If your pharmacy already holds the prescription, it can transfer it.16
  3. Wait for the text. "After the prescription is received and processed, you'll get a text with next steps to set up your account, pay, and choose delivery or pickup."16
  4. Pay online by card, Apple Pay, Google Pay, or HSA or FSA card. No coupon needed.16
  5. Schedule delivery or pickup. Delivery is free; you get tracking once it ships.16 If you started on the $199 offer, your third box is $349.13

LillyDirect and NovoCare against the compounded floor

The makers' prices are the floor for FDA-approved, brand-name tirzepatide and semaglutide. They are not the floor for the molecule. Compounded tirzepatide on our board starts at $85 a month all in, and compounded semaglutide at $69, with the prescriber included. That is less than LillyDirect's lowest Zepbound price, and less than NovoCare's standard Wegovy price.

The trade-off is approval. The FDA is direct about it: "compounded drugs are not FDA approved. This means the agency does not review compounded drugs for safety, effectiveness or quality before they are marketed."24 The makers' channels sell the approved product; compounded programs sell a pharmacy-made version at a lower price. Both are legal routes in the US today, and which one fits is a decision for you and your prescriber. Our guides on compounded tirzepatide and whether compounded semaglutide is safe cover the differences.

And branded Zepbound sits where on our board? The cheapest program selling it charges $348 a month all in, prescription included, which is above LillyDirect's 2.5 mg price, because it includes the clinician. For Wegovy the cheapest program on our board is $208 a month all in.

Which one should you use?

The pharmacy follows from the drug, so start there. Then:

  • Your prescriber chose Zepbound and you pay cash: LillyDirect, or the KwikPen with Lilly's card at a retail pharmacy. Keep every refill inside 45 days once you reach 7.5 mg; it is the difference between $449 and $699 a month at the top doses.3
  • Your prescriber chose Wegovy and you pay cash: NovoCare Pharmacy. Start before 31 December 2026 if you can, to get the two $199 boxes.13 If you are sure you will stay on it a year, compare the telehealth subscription quoted above against NovoCare's $349.22
  • You want a pill: both start at $149.1115 The ladder slots above show where each tops out.
  • You have Medicare Part D and qualify for the Bridge: $50 through your plan beats both cash counters. For Zepbound that means the KwikPen, not the vial.23
  • You have commercial coverage for the drug: neither counter. Use your plan with the maker's copay card; see GLP-1 copay cards.
  • You have no prescription yet: a program on our board bundles the visit and the drug in one monthly price. Start with the ranking below.

Every route is compared in one place in GLP-1s without insurance, and the government price list is in TrumpRx for GLP-1s.

Common questions

How much is Zepbound at LillyDirect?

Zepbound vials and the KwikPen cost $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5, 10, 12.5 and 15 mg on LillyDirect, as long as each refill is bought within 45 days of your last delivery. Late refills cost $499 at 7.5 mg and $699 at 10 to 15 mg. Prices read at Lilly's own pages on 15 October 2026.

How much is Wegovy at NovoCare Pharmacy?

NovoCare Pharmacy charges $349 a month for every Wegovy pen dose from 0.25 to 2.4 mg, $399 for Wegovy HD 7.2 mg, and $199 for a new patient's first two boxes at 0.25 or 0.5 mg (prescriptions written by 31 December 2026). The Wegovy pill costs $149 at 1.5 mg, $199 at 4 mg and $299 at 9 or 25 mg.

What is Lilly's 45-day rule for Zepbound?

From 7.5 mg up, Lilly gives you the $449 price automatically on your first order and keeps it only if each refill is completed within 45 days of the date your previous order was delivered. Miss it and that month costs the regular price: $499 at 7.5 mg, $699 at 10, 12.5 or 15 mg. The clock starts at delivery, not at your last dose.

Does NovoCare Pharmacy take insurance?

No. NovoCare Pharmacy is a cash, self-pay pharmacy, and its terms say insurance is not accepted. What you pay does not count toward your deductible or out-of-pocket limit. If your plan covers Wegovy, the Wegovy savings offer at a regular pharmacy can bring the copay as low as $25, with a $100 monthly cap.

Can I use NovoCare or LillyDirect if I have Medicare?

NovoCare says government-insured patients may self-pay outside their insurance. Lilly's savings card terms exclude government beneficiaries, so ask LillyDirect at checkout. Since 1 July 2026, Medicare's GLP-1 Bridge charges $50 a month for Wegovy (pen or pill), Foundayo and the Zepbound KwikPen, but not Zepbound vials, if you meet the criteria.

Is Zepbound cheaper than Wegovy if I pay cash?

Only at the start. Zepbound's 2.5 mg month is $299 against Wegovy's $349 standard price (or $199 for a new patient). From the second strength up, Wegovy at NovoCare costs $349 a month against Zepbound's $399 and $449, and Wegovy HD is $399. A late Zepbound refill at 10 mg or above costs $699.

Does LillyDirect sell the Zepbound KwikPen or only vials?

Both. Since 23 February 2026, LillyDirect sells the four-dose Zepbound KwikPen and single-dose vials at the same self-pay prices. The KwikPen can also be bought at major retail pharmacies at the same price with Lilly's KwikPen Self-Pay Savings Card; vials stay with LillyDirect and its Walmart pickup.

Is shipping free from LillyDirect and NovoCare?

Yes. Lilly says medicines filled through LillyDirect ship free of charge, and NovoCare says its shipping and delivery are free. Both also offer pickup: Walmart pharmacies for LillyDirect vials, and a transfer to your local pharmacy for NovoCare.

Who fills NovoCare Pharmacy orders?

Novo works through licensed partner pharmacies. CoAssist Pharmacy takes in the prescription, and orders may be filled by CenterWell Pharmacy or a pharmacy in the BlinkRx network, which is licensed in all 50 states, the District of Columbia and Puerto Rico. Expect texts from those names.

Can I use HSA or FSA money at NovoCare Pharmacy?

Yes. NovoCare Pharmacy accepts credit cards, Apple Pay, Google Pay and FSA or HSA cards. The purchase still does not count toward your insurance deductible.

Is compounded tirzepatide cheaper than LillyDirect?

Usually. Compounded tirzepatide on our board starts at $85 a month all in, with the prescriber included, against LillyDirect's $299 to $449 for Zepbound. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality before they are sold.
