Costco does not set its own GLP-1 price. Its pharmacy sells Wegovy and Ozempic at Novo Nordisk's self-pay price, run through Costco's member discount program. That price was $499 a month when the deal launched in October 2025,1 and Novo's own pharmacy now charges $349 a month for every Wegovy pen dose from 0.25 mg to 2.4 mg, after two $199 starter months for new patients.3
Three more facts most "Costco GLP-1" pages miss, each read on 15 October 2026. The 2024 Costco and Sesame weight-loss bundle no longer appears on Sesame's Costco page.11 Costco's member drug discount cannot be stacked with insurance, Medicare or another discount card.89 And CVS, Walgreens and Walmart now pass through the same maker prices: $149 for the Wegovy pill's starting dose1516 and $299 for a starting Zepbound vial collected at Walmart.1718
Novo Nordisk's self-pay price for a month of Wegovy pens at any dose from 0.25 mg to 2.4 mg, after two $199 intro months for new patients. Costco's October 2025 launch price was $499.
Below: every store's price in one dated table, how the Costco and Sesame tie-up works today, what CVS, Walgreens, Walmart and Sam's Club charge, the maker prices they all match, a year of costs worked through, and the questions to ask at any pharmacy counter before you pay.

Read the table by the third column, then the fourth. The prices cluster because they are the same prices: the makers set a self-pay figure, and the chains run it at the counter. What differs is the visit you need to get a prescription, the membership, and whether a store will process the maker's offer for your drug and dose. Telehealth programs sit on top of these prices with their own fees; the live ranking for those is further down.
What Costco actually sells: the Novo deal, explained
In October 2025, Novo Nordisk and Costco announced that Costco members paying cash could buy Wegovy and Ozempic at Costco pharmacies. The reported terms, in FOX Business's words:1
The medication will be available through the Costco Member Prescription Program and will cost $499 for a four-week supply, according to reports. Novo Nordisk already offers the medication at that price to self-paying patients through its website.
FOX Business, 4 October 2025
The second sentence is the one to hold on to. The $499 was never a Costco discount on a Costco price; it was Novo's self-pay price, made available at a Costco counter. TODAY put it at four injectable pens for $499, about half of paying out of pocket at the time.2 To qualify, members needed a prescription and had to pay out of pocket rather than through insurance.1
Six weeks later the number underneath moved. Novo's own pharmacy now lists Wegovy pens at $349 a month for 0.25 mg through 2.4 mg, and Ozempic pens at $349 for 0.25 mg, 0.5 mg and 1 mg, with $499 kept only for Ozempic 2 mg.3 New patients pay $199 for each of their first two months at the two lowest pen doses, an offer that runs through 31 December 2026.3
What that means at the Costco counter. We could not read a current Costco GLP-1 price online: Costco's drug price lookup did not render a GLP-1 figure to us on 15 October 2026, and Costco has not published an updated number we could cite. Novo's terms say its savings offer may be redeemed at participating retail pharmacies,6 so the practical move is to ask the Costco pharmacist to run the Wegovy or Ozempic self-pay option at your dose and compare it with the member discount price. If a Costco counter quotes $499 for a 1 mg pen, ask whether that is the October 2025 figure.
Ozempic self-pay is for its labelled use. Novo's programme terms say a patient may use it only if Wegovy or Ozempic has been prescribed for an FDA-approved indication within that product's labeling.5 Ozempic's label covers type 2 diabetes; Wegovy is the semaglutide labelled for weight.
Does Costco carry Zepbound?
Lilly says self-pay pricing on the Zepbound KwikPen is available at LillyDirect and "major pharmacies nationwide", from $299 a month for the 2.5 mg dose, through a KwikPen self-pay savings card.19 Lilly does not list the chains by name in that announcement, and we could not confirm a Costco-specific Zepbound price. Ask the Costco pharmacy whether it accepts the Zepbound KwikPen self-pay card. The card applies only to the KwikPen, not to vials or the single-dose pen.19
The Costco Member Prescription Program, read from Costco's own FAQ
"Costco GLP-1" searches often land on this programme, so here is what it is and is not, in Costco's words.8
The Costco Member Prescription Program is NOT insurance. It is a prescription drug discount card program that provides eligible Costco members and their eligible dependents with the ability to obtain lower prices at participating pharmacies.
Costco, Member Prescription Program FAQ
- Cost: no additional charge; it comes with a Costco membership, and your membership card is the discount card.8
- Size of discount: Costco puts the savings at 2% to 40% or more depending on the drug, decided at the time of purchase.8
- Where it works: Costco pharmacies, Costco mail order and participating neighbourhood pharmacies, and Costco names Walgreens, Albertsons, Safeway and Vons among them.8
- Insurance: you choose the discount card or your drug plan for a fill. Money spent through the discount card does not count toward your plan's deductible.8
- Stacking: other discount cards cannot be used together with the card's pricing, and it cannot be combined with any commercial, Medicare Part D or state plan.9
- Medicare and Medicaid: you must choose either the card or the government programme for a prescription.9
The stacking rule is the one that matters for a GLP-1. A manufacturer self-pay offer and the member discount are two different price routes, and Costco's terms do not let you pile one discount card on another. Ask for both numbers and pay the lower one.
Same principle, Novo's version: paying outside your insurance means that money does not count toward your out-of-pocket limit or deductible.4 If your plan covers your GLP-1 and you are close to your deductible, run the insured price too.
Costco and Sesame: what the partnership offers now
The "Costco Sesame" search goes back to April 2024, when Costco began selling a medical weight-loss programme run by Sesame, a cash-price telehealth marketplace. WebMD reported the launch terms:10
For $179, subscribers receive 3 months of medical weight loss services from Costco’s partner, Sesame, building on a relationship with other fee-based services like primary care visits. The fee covers clinical meetings with a provider who is qualified to write prescriptions for weight loss medications, but the cost of any prescriptions would not be covered under the fee.
WebMD, 2 April 2024
That three-month bundle is not on Sesame's Costco page in October 2026. The page now lists these member prices:11
Costco Members receive Sesame’s best pricing on a range of health services across all 50 states, including: Virtual primary care, just $29. Mental health medication management, just $65/mo. Health check-ups (a standard lab panel, plus a virtual follow-up consultation with a provider), just $72. Virtual mental health therapy, just $79. Plus, 10% off of all other Sesame services, including in-person appointments.
Sesame, Costco member page, read 15 October 2026
No weight-loss line appears in that list. Whether the member discount on "all other Sesame services" extends to Sesame's weight-loss subscription is not stated on either page, so ask Sesame's Costco support line before you sign up. To use member pricing at all, you create a Sesame account and verify your Costco membership through your Costco.com account.11
Sesame's weight-loss plan, for anyone
Sesame now sells weight-loss care as Success by Sesame, open with or without a Costco card. The care subscription runs $59 a month on a 12-month commitment, and the medication is billed separately.12 Sesame's own plan terms:
The Success by Sesame subscription starts at just $59 per month with an annual subscription and includes telehealth visits, unlimited messaging, and ongoing clinical care from a provider of your choice. The subscription fee does not include the cost of medication. Month-to-Month Plan: Billed every month at $99 per month.
Sesame, read 15 October 2026
Its prescribers write brand drugs at the makers' cash prices: Sesame lists the Wegovy pill from $149 a month and the Zepbound KwikPen from $299, each billed on top of the care fee.13 All in, care plus drug, the Wegovy pill through Sesame comes to $208 a month at the starting dose on our board, and Sesame accepts insurance for the medication if your plan covers it.13

Sesame's full plan breakdown, including the shorter terms, lives on its Sesame Care price page. The ranking below shows where it sits among every program selling Wegovy today.
Ozempic at CVS: what the counter and MinuteClinic charge
CVS rebuilt its weight-loss offer on 5 August 2026. Three numbers came out of it, in CVS's own release.1415
- $29 for a MinuteClinic online visit, with no membership or recurring fee, where a clinician can evaluate you and, where clinically appropriate, prescribe a GLP-1.14 CVS says the programme is designed for self-paying adults aged 18 to 64 who are overweight or living with obesity.15
- As low as $149 without insurance at CVS Pharmacy when a manufacturer voucher applies to a qualifying drug and dose, and as low as $25 with commercial insurance plus a manufacturer coupon.15
- The $50 Medicare GLP-1 Bridge, which CVS Pharmacy participates in.15
For Ozempic specifically, the CVS counter price is Novo's self-pay price when the pharmacy runs it: $349 a month for 0.25 mg, 0.5 mg or 1 mg, $499 for 2 mg, and $199 for each of the first two months at the lowest doses for new patients.3 The Ozempic pill starts at $149 a month.4 CVS says it accepts third-party discount cards, manufacturer coupons and manufacturer vouchers,15 so the counter can price more than one route for you.
Zepbound and Foundayo are the next step at CVS. By early in the fourth quarter of 2026, CVS says eligible Zepbound and Foundayo patients will see transparent pricing, including cash-pay options, in the CVS Health app, with pickup as early as the same day at its 9,000 locations.14 The release also notes that both oral and injectable Wegovy are already stocked.14
Walgreens Ozempic and Wegovy cost
Walgreens launched Walgreens Weight Management on its Virtual Healthcare platform on 26 February 2026. Each visit costs $49, with no monthly subscription, and the prescription can go to the pharmacy of your choice.16 Like CVS's, it targets adults aged 18 to 64 who plan to self-pay, and Chain Drug Review reported it available in most states at launch.16
On the drug side, Walgreens added Novo's prices to its Rx Savings Finder tool: $149 a month for the Wegovy 1.5 mg or 4 mg tablet and $199 a month for the 0.25 mg and 0.5 mg pens, subject to Novo's terms.16 Those were Novo's prices in February. NovoCare's current list keeps the 1.5 mg tablet at $149 but shows the 4 mg tablet at $199, and moves pens to $349 after the two intro months,3 so check the tool at your dose rather than the launch figures.
Walgreens is also inside Costco's network. Costco names Walgreens among the participating neighbourhood pharmacies where its member discount card works.8 For a GLP-1 that is one more price to ask for, not a third discount to add: the card cannot be combined with another discount card on the same fill.9
Ozempic cost at Walmart, and Zepbound pickup
Walmart's GLP-1 story is Lilly's, not Novo's. Since mid-November 2025, Walmart pharmacies have been the first in-store pickup point for LillyDirect's self-pay Zepbound vials:17
Walmart, with nearly 4,600 pharmacies nationwide, will serve as the first in-store pickup pharmacy for LillyDirect’s self-pay single-dose vials.
Walmart and Eli Lilly, 29 October 2025
You need a valid, on-label prescription and you pay without insurance.17 The launch release priced the lowest dose at $349. Lilly cut vial prices on 1 December 2025, so the current ladder is lower:18
| Zepbound vial dose | Self Pay Journey Program price | Regular price outside the program |
|---|---|---|
| 2.5 mg | $299 | $299 |
| 5 mg | $399 | $399 |
| 7.5 mg | $449 | $599 |
| 10 mg | $449 | $699 |
| 12.5 mg | $449 | $849 |
| 15 mg | $449 | $1,049 |
Source: Lilly's 1 December 2025 release.18 The gap in the right-hand column is the reason to stay inside the Self Pay Journey Program: at 15 mg it is $600 a month.
For Ozempic and Wegovy at Walmart, no Walmart-specific price was published that we could cite. Ask the pharmacist to run Novo's self-pay option; Novo's savings offer is redeemable at participating retail pharmacies, and its self-pay pen price is $349.36
Sam's Club
Walmart's warehouse club launched its own Novo arrangement the same week as Costco. TODAY reported that Sam's Club shoppers could get a month of Ozempic or Wegovy for $499 using Novo Nordisk co-pay cards.2 Same-day delivery of the pens cost $12.2 As with Costco, that $499 matched Novo's self-pay price at the time, and Novo's pen price has since fallen to $349.3
The maker prices every store is matching
Behind every chain number on this page sit two lists: NovoCare Pharmacy for Wegovy and Ozempic, and LillyDirect for Zepbound and Foundayo. Both makers also sell direct, with home delivery. Our side-by-side of the two direct pharmacies is in LillyDirect vs NovoCare. The ladders below are the manufacturers' own, with the cheapest tracked seller at each step.

Source: NovoCare Pharmacy, read 15 October 2026.34 One month means one bottle of 30 tablets or one box of four Wegovy pens.3
Lilly's Foundayo tablet joined the pharmacy shelves in 2026. Amazon Pharmacy prices it at $149 a month for cash-pay customers, with manufacturer coupons applied automatically at checkout.20
A year of Wegovy, worked through by store
Monthly prices hide the shape of a year. Here is the arithmetic on the published figures, before any visit fee, for someone paying cash.
| Route | Months 1 and 2 | Months 3 to 12 | First year, drug only |
|---|---|---|---|
| Pen at a store running the maker's self-pay price, as a new patient | $199 each | $349 each | $3,888 |
| Pen at the October 2025 Costco or Sam's Club launch price | $499 each | $499 each | $5,988 |
| Pill held at 1.5 mg all year | $149 each | $149 each | $1,788 |
| Pill held at 9 mg or 25 mg | $299 each | $299 each | $3,588 |
Prices from NovoCare3 and the October 2025 launch reports.12 The pen rows assume the dose stays inside the $349 band; Wegovy HD is priced separately. Your prescriber sets the dose, and a pill held at one strength all year is an arithmetic case, not a plan.
Then add the cost of the prescription itself. At list prices published by each chain, a year of monthly check-ins costs $348 at $29 a visit and $588 at $49 a visit,1416 and most people will not need a visit every single month. A telehealth care subscription such as Sesame's adds its fee every month whether or not you see anyone; on Sesame's annual plan that is $708 for the year.12
The difference between the two pen rows, $2,100 a year, is the cost of reading a 2024 or 2025 article and paying its number.
Where telehealth programs fit against the store counter
A pharmacy counter sells you the drug; you still need someone to prescribe it. Telehealth programs bundle the two, and some also sell compounded semaglutide or tirzepatide, which a retail chain does not stock. Compounded drugs are not FDA-approved, a point Novo prints at the top of its own pharmacy page.22 Weigh that before comparing on price alone.
Across every program we track, the cheapest GLP-1 is $69 a month all in today, and the cheapest Zepbound program is $348. The chart shows every program's all-in monthly price, read off each seller's own page.

On Medicare? The $50 Bridge works at the pharmacy counter
If you have Medicare Part D drug coverage and meet CMS's criteria, the Medicare GLP-1 Bridge changes every number above. Medicare.gov lists the drugs and the copay:21
Starting July 1, 2026, Medicare covers these GLP-1 drugs: Foundayo® (tablet) Wegovy® (injection or tablet) Zepbound® (KwikPen® only.
Medicare.gov
You pay a $50 copay to the pharmacy for a one-month supply, 28 or 30 days depending on the drug.21 CVS Pharmacy says it participates.15 Two rules from earlier on this page apply here: Costco's member discount cannot be combined with Medicare Part D,9 and the Zepbound vials picked up at Walmart are not Bridge drugs, because Medicare covers the KwikPen only.21 Our Medicare GLP-1 guide covers who qualifies and what the $50 does not count toward.
How to get the right price at any pharmacy counter
Novo publishes a script for this. Before you pick up a prescription at a retail pharmacy, it suggests asking the pharmacist to compare every pricing option, in three questions:7
- Insurance: does my insurance cover it, and what would my monthly cost be before and after my deductible?
- Savings offer: do I qualify for the maker's savings offer, and what is the price with it applied?
- Self-pay: can you process this prescription on the maker's self-pay option, and what is the monthly price at my current dose?
At Costco, add a fourth: what is the Member Prescription Program price for this drug and dose?8 Then pick one route for the fill, since Costco does not let its discount card be combined with insurance or another discount card.9
Three more checks that save a second trip:
- Name the form. Lilly's retail self-pay card covers the Zepbound KwikPen only, not vials or single-dose pens.19 Walmart pickup is for vials.17
- Name the dose. Novo's prices step by dose: the Wegovy pill is $149 at 1.5 mg and $299 at 9 mg.3
- Ask about the intro months. Novo's $199 starter price on the two lowest pen doses applies to new patients for two monthly fills, through 31 December 2026.3
