With a type 2 diabetes diagnosis this is an ordinary prior authorisation: your prescriber documents the diagnosis and submits it. Because there is no benefit-exclusion flag on the diabetes listing, your plan design is far less likely to be the obstacle than it would be for a weight-loss request.
