That is why two people holding identical cards get opposite answers, and why the useful question for member services is whether your plan includes the weight-loss medication benefit at all, before anything about criteria.

Get your real answer
For diabetes, have the diagnosis documented and submit the prior authorisation in the ordinary way. If weight is the goal, ask about Zepbound rather than Mounjaro, and expect the benefit-exclusion question to come first: check Price a Medication in your member portal and ask HR whether the weight-loss benefit is included.
