The Zepbound KwikPen is one pen that holds a month of Zepbound: four fixed weekly doses of the same strength. It is the presentation Medicare's GLP-1 Bridge covers and one you can buy for cash through LillyDirect,811 and it works differently from the single-dose pen most people picture: you screw on a needle, prime it, dial, inject, and do that four times before the pen goes in the bin.
The KwikPen at a glance
| Question | What the label says |
|---|---|
| Doses per pen | 4, "one dose taken weekly"3 |
| Strengths | 2.5, 5, 7.5, 10, 12.5 or 15 mg per dose; one strength per pen2 |
| Volume | 2.4 mL per pen, 0.6 mL per dose2 |
| Needles | "KwikPen-compatible Pen Needle (not included)"; a new one each week34 |
| Setting the dose | Turn the knob "all the way until it stops"; the doses are fixed, there is nothing to measure3 |
| Priming | Before every weekly injection4 |
| Once in use | Fridge or room temperature up to 86°F; out after 30 days or 4 doses, whichever comes first6 |
| Leftover medicine | Thrown away with the pen; never drawn into a syringe3 |
The dose on a KwikPen is not adjustable. The 10 mg pen gives 10 mg each time you turn the knob to its stop, four times. Moving up or down a dose means a new pen at the new strength, on your prescriber's instruction. The full ladder, with the label's escalation rule, is in the Zepbound dosage chart.
KwikPen, single-dose pen or vial
The label lists four ways Zepbound comes: "pre-filled single-dose pens, single-dose vials, multi-dose vials, or single-patient-use KwikPens", and "the multi-dose vials and single-patient-use KwikPen each contain 4 doses".1 The same molecule and the same strengths in every one. What changes is how you inject it and who pays for it.
| Presentation | Doses | What you need | Medicare GLP-1 Bridge |
|---|---|---|---|
| KwikPen | 4 per pen | Pen needles, one per week | Covered8 |
| Single-dose pen | 1 per pen, 4 pens per carton | Nothing extra | Not covered8 |
| Single-dose vial | 1 per vial | A syringe and needle | Not covered8 |
| Multi-dose vial | 4 per vial | A syringe and needle, 0.6 mL a dose | Not covered8 |
Next to the single-dose pen, the KwikPen adds three steps: attach a needle, prime, dial. Next to a vial it removes the one that goes wrong most, measuring a volume into a syringe. The label adds one limit: the KwikPen "is not recommended for self-administration by those who are visually impaired".7
What a KwikPen costs
Paying cash, the KwikPen is priced by strength, and Lilly's self-pay ladder sets the reference. Here it is with the date we last checked it, beside the cheapest verified seller at each step:
The KwikPen is also listed on TrumpRx, the federal price site, which hands you to LillyDirect: "LillyDirect® will guide you to savings options, eligibility confirmation, and offers delivery options or transfer to a retail pharmacy of your choosing."11 The same page tells insured readers to "check your co-pay first".11
Branded Zepbound through the telehealth programs on our board starts at $348 a month today, which includes the program's own consultation and membership costs. Ranked by all-in price:
One pen is one month. A pen holds four weekly doses and is thrown away after the fourth, so a year on a steady dose is 13 pens, not 12. Budget for pen needles as well, since none come in the carton.3 If price is the deciding factor, compounded tirzepatide, which is made by a licensed pharmacy and is not FDA-approved, starts at $85 a month on our board; Zepbound without insurance sets every route side by side.
Medicare covers the KwikPen, and only the KwikPen
This is the detail that catches people. From 1 July 2026, Medicare's GLP-1 Bridge covers Zepbound for weight loss, but Medicare.gov is specific: "Zepbound® (KwikPen® only. The program doesn’t cover single-dose Zepbound® vials or pens)."8 A prescription for the single-dose pen will not go through the Bridge. Ask for the KwikPen by name.
What it costs: "a $50 copayment to the pharmacy for a one-month supply (either 28 or 30 days, depending on drug)". That copay "doesn’t count towards your yearly Medicare drug plan deductible or out-of-pocket limit".8 At one pen a month, a year is 13 pens, $650 in copays. To qualify you must be 18 or older and meet one of Medicare's BMI routes when you start, the simplest being a BMI of 35 or higher; lower thresholds apply with specific conditions.8 The Bridge runs "through Dec. 31, 2027".9
After that, the BALANCE model is scheduled to take over, and the KwikPen stays the Zepbound in it: KFF lists "all formulations of Foundayo, Mounjaro, Ozempic, Rybelsus, and Wegovy, and the KwikPen formulation of Zepbound".10 The full eligibility list, and how prior authorization works, is in does Medicare cover GLP-1s.
With commercial insurance
Plans decide coverage by presentation, not just by drug, so "Zepbound is covered" can still mean the KwikPen is not. Before you fill, check that the exact form on your prescription, KwikPen, single-dose pen or vial, is the one your plan lists. Our coverage checker has the insurer-published detail we have read, plan by plan, and the Zepbound savings card guide covers Lilly's copay card.
Needles: what to get
The carton holds the pen and no needles. The label's supply list is "KwikPen-compatible Pen Needle (not included)", plus an alcohol swab, gauze or cotton and a sharps container.3 Ask your prescriber to add pen needles to the prescription, or ask the pharmacist which compatible needles they stock when you collect the pen. You need one per injection: "Always use a new pen needle for each injection to help prevent infections and blocked needles."4 Four weeks on one pen is four needles, plus any you change during priming.
Used needles go straight into a sharps container; the pen itself, needle removed, can go in household trash or the sharps container.5 And unscrew the needle after each injection: the label says not to "store the Pen with the needle attached to prevent leaking, blocking the needle, and air from entering the Pen".5
How to use it, in the label's words
The Instructions for Use run to 17 steps. The four that are different from a single-dose pen:
- Prime every week. "Prime before each weekly injection." Turn the knob "until you hear 2 clicks and the extended line is shown in the dose window", point the needle up, push the knob in. "Your Pen has been primed if a small amount (a drop) of medicine comes out of the tip of the pen needle."4
- Dial to the stop. "Turn the dose knob all the way until it stops AND the icon is in the dose window. This is your weekly dose. Do not count clicks as you select the dose."4
- Hold for five. Push the knob in "until it stops then slowly count to 5 while holding the dose knob".4
- Check the icon. If it is not in the window, "insert the needle back into your skin and finish your injection. Do not redial the dose." Unsure you got it all: "do not start over or repeat the injection."5
Injection sites are the thigh or stomach, at least 2 inches from the belly button, or the back of the upper arm if someone else injects.4 The injection guide covers technique. Lilly's helpline for a pen that will not prime is on the label: 1-800-LillyRx.4
Storage and the leftover medicine
Unopened, the KwikPen lives in the fridge until its expiry date; left unopened at room temperature, it is thrown away after 30 days. Once you start it, keep it in the fridge or at room temperature up to 86°F, and throw it away "after a total of 30 days at room temperature, 30 days after first use, or after taking 4 weekly doses, even if there is medicine left in it".6
There will be medicine left. The pen "will prevent you from dialing a full dose after you have given yourself 4 weekly doses", and the label is blunt about the rest: "Do not inject the leftover medicine. Do not transfer ZEPBOUND from your Pen into a syringe."3 Scraping a fifth dose out of the pen is the one shortcut the label rules out by name.
Never share a pen, "even if the pen needle is changed. Sharing poses a risk for transmission of blood-borne pathogens."7
The short version
One KwikPen is one month: four fixed doses, one strength, a new needle and a prime every week, and in the bin after the fourth dose or 30 days. It is the only Zepbound Medicare's Bridge pays for, at $50 a month through 2027. Paying cash, Lilly's ladder sets the reference above, and the programs on our board start at $348 a month for branded Zepbound today.
