Compounded Semaglutide | Week 1 & 2 Update | Weight Loss

Patient storyProject Hellion35,659 views7:47Jun 2023

Our summary

In this update, the creator describes her first two weeks on compounded semaglutide at 10 units, recorded right after her third injection. She says she lost 9.8 pounds in that time, more than she has ever lost in two weeks, and she expects part of it to be water weight. She also says she feels less bloated and has lost a couple of inches.

What else the video covers

She walks through how appetite changed across each week. Her first shot was at 10:30 in the morning, and she says she did not notice appetite suppression until about 6:30 that evening. For roughly the first three to four days after each shot she felt almost no hunger, and the thought of food made her queasy, so she ate very small, high protein portions several times a day. Hunger returned gradually, and she says she was hungriest on the sixth and seventh days of each week. Even then she says she gets full quickly and eats far less than she used to.

She reports no major side effects beyond the queasiness early in each cycle. She says she has had no headaches, no trouble in the restroom and no vomiting, and she drinks electrolytes and high protein shakes. She calls the injections painless. Her fifth shot would normally move up to 15 units, but she says that if her results stay strong she plans to ask her provider whether she needs to increase at all, and she will post a follow-up once she decides between 10 and 15 units.

Our guides closest to what this video covers, with prices checked weekly.

  • Semaglutide side effects, and what compounding does and does not change

Semaglutide's side effects are gastrointestinal: nausea, vomiting, diarrhoea and constipation, worst early and after each dose increase. Those belong to the molecule, so they are the same whether it came in a Novo Nordisk pen or a compounding pharmacy's vial. What compounding changes is everything around the molecule. A compounded drug is not FDA-approved, so the finished product has not been reviewed for safety, effectiveness or manufacturing quality, and the FDA has said the enforcement discretion that allowed 503A pharmacies to compound semaglutide has ended. It also usually arrives as a vial and a syringe rather than a pre-set pen, which moves dose measurement to you.

  • Semaglutide for weight loss: which version, and what each costs

Semaglutide is the molecule. For weight loss the FDA-approved version is Wegovy, sold as a weekly pen and a daily tablet. Ozempic is the same molecule approved for type 2 diabetes, so using it for weight is off-label. Compounded semaglutide is the same molecule from a compounding pharmacy, is not FDA-approved, and is what most cash buyers actually take because it costs a fraction of the brand. All three produce the same side effects. What differs is the approval, the oversight and the price.

  • Stopping a GLP-1: what the withdrawal trials found, and what a maintenance dose actually is

Two trials have measured what happens when you stop. On semaglutide, people who lost 17.3% over 68 weeks regained two-thirds of it in the year after stopping, ending 5.6% below where they started. On tirzepatide, people who lost 20.9% in 36 weeks regained 14% over the next year on placebo while those who continued lost a further 5.5%. Both labels are explicit that these drugs are for maintaining weight reduction long term, which is a different claim from finishing a course. A maintenance dose is simply the dose you stay on; the labels do not define a lower one to step down to at goal.
