Upped my Semaglutide (Glp1) maintenance dose only 5 units! Did it make me sick?

Patient storyKarli Sine6,037 views2:25Dec 2023

Our summary

Karli Sine says she had been taking a steady 15 units of semaglutide as her maintenance dose. Because she has a trip planned for February, she decided to raise it slightly, and she took 20 units, an increase of only 5 units. She says she then got sick.

One person's experience, in their words. Not medical advice.

What else the video covers

She describes the sequence: she had a somewhat larger dinner than usual, went straight home, and felt a little off without connecting it to the dose. The next morning she woke with a very gurgly stomach and sulfur burps and expected to throw up. She says she drank a lot of water, took Zofran, and later sipped a protein shake, which she says helped a great deal, though she notes the Zofran had probably started working by then. She adds that she tends to get sick two days after her shot, "like clockwork," and asks viewers whether that happens to them, since she knows some people feel unwell the day of the shot or the day after.

She says she feels better now and plans to stay on 20 units to see how it goes, which she describes as 20 units once a month. If she is not happy with it, she says she may go back down. She says it took her three to four weeks to feel normal when she first started the medication. She closes by asking viewers what helps them when they are sick and mentions a link in her profile with a code for half off a telehealth consult.

Questions this video answers

Can a small semaglutide dose increase make you sick?

Karli says she went from 15 units to 20 units, an increase of 5 units, and then felt sick. She also notes she had a larger dinner than usual that night, so she does not attribute it to the dose alone.

What did she do to feel better after getting sick on semaglutide?

She says she drank a large amount of water, took Zofran, and sipped some of a protein shake. She says the protein shake helped her a lot, though she thinks the Zofran had likely started working too.

How long does semaglutide sickness last for her?

Karli says she gets sick two days after her shot, which she describes as happening like clockwork. She also says it took her three to four weeks to feel normal when she first started taking the medication.

  • Stopping a GLP-1: what the withdrawal trials found, and what a maintenance dose actually is

Two trials have measured what happens when you stop. On semaglutide, people who lost 17.3% over 68 weeks regained two-thirds of it in the year after stopping, ending 5.6% below where they started. On tirzepatide, people who lost 20.9% in 36 weeks regained 14% over the next year on placebo while those who continued lost a further 5.5%. Both labels are explicit that these drugs are for maintaining weight reduction long term, which is a different claim from finishing a course. A maintenance dose is simply the dose you stay on; the labels do not define a lower one to step down to at goal.

  • Semaglutide side effects, and what compounding does and does not change

Semaglutide's side effects are gastrointestinal: nausea, vomiting, diarrhoea and constipation, worst early and after each dose increase. Those belong to the molecule, so they are the same whether it came in a Novo Nordisk pen or a compounding pharmacy's vial. What compounding changes is everything around the molecule. A compounded drug is not FDA-approved, so the finished product has not been reviewed for safety, effectiveness or manufacturing quality, and the FDA has said the enforcement discretion that allowed 503A pharmacies to compound semaglutide has ended. It also usually arrives as a vial and a syringe rather than a pre-set pen, which moves dose measurement to you.

  • GLP-1 constipation, nausea, diarrhoea and fatigue: the label numbers, and what the labels say to do

The four gut effects are the four most common side effects of every GLP-1, and the labels put numbers on them. On Wegovy 2.4 mg: nausea 44%, diarrhoea 30%, constipation 24%, vomiting 24%, fatigue 11%, against 16%, 16%, 11%, 6% and 5% on placebo. On Zepbound at 15 mg: nausea 28%, diarrhoea 23%, constipation 11%, vomiting 13%, fatigue 7%. They cluster around dose increases, they are the reason 6.8% of Wegovy patients and up to 6.7% of Zepbound patients stopped, and both labels prescribe the same fix: climb slower.
