SEMAGLUTIDE: 8 MONTH UPDATE | Weight Loss at Age 49

Patient storyGeek Out of Water38,242 views38:15Aug 2024

Our summary

Shelly, who runs the Geek Out of Water channel, gives an 8 month update on taking compounded semaglutide. She says she started just after Christmas 2023 at 274 lbs, her highest weight ever, and is now at 230 lbs, a loss of 44 lbs or 16% of her body weight. She reports being at what she calls her max dose of 2.5 mg per week, a self-injection, and says she pays $131 per milliliter, about $2,000 for a 15 milliliter supply, through a med spa. She says this is not medical advice and points viewers to an FDA warning about compounded semaglutide.

One person's experience, in their words. Not medical advice.

What else the video covers

On side effects, she says the early mild nausea faded and that constipation appeared only at the max dose, which she manages by eating vegetables daily. She says the food noise is still completely gone. She estimates her loss at about 1.37 lbs per week on average and says her scale shows muscle mass going from about 110 lbs to 108 lbs, or roughly 40% to 47% of body weight. She describes walking as her main exercise and says she is not yet doing strength training. She also says loose skin comes from weight loss itself and not from the drug.

She describes taking a half dose for one week without telling her provider, to stretch a vial, and says the food noise returned strongly and left her close to tears. She also talks about learning to trust hunger cues, eating roughly 1,200 to 1,800 calories a day without counting, and her goal of around 175 lbs, which she says she will discuss with her doctor.

Questions this video answers

How much weight did she lose on semaglutide in 8 months?

Shelly says she went from 274 lbs to 230 lbs, a loss of 44 lbs or 16% of her body weight, over 32 weeks. She says the pace averaged about 1.37 lbs per week and has slowed from about 1.75 lbs per week early on.

What happened when she lowered her semaglutide dose?

She says she took a half dose for one week to stretch a vial, without clearing it with her provider. She reports that on the day before her next shot the food noise returned strongly and was distressing, and it took a few days after the full dose for relief to come back.

How much does compounded semaglutide cost through a med spa?

Shelly says she pays $131 per milliliter and buys 15 milliliters at a time, which comes to about $2,000 per order. She says she can barely afford it and that much of it goes on credit cards.

Does semaglutide cause muscle loss according to this video?

Shelly says muscle loss is a concern with any weight loss. She reports her scale showed about 108 lbs of muscle versus roughly 110 lbs at the start, and she credits protein intake, noting she has been vegetarian since 2003.

Our guides closest to what this video covers, with prices checked weekly.

  • Semaglutide side effects, and what compounding does and does not change

Semaglutide's side effects are gastrointestinal: nausea, vomiting, diarrhoea and constipation, worst early and after each dose increase. Those belong to the molecule, so they are the same whether it came in a Novo Nordisk pen or a compounding pharmacy's vial. What compounding changes is everything around the molecule. A compounded drug is not FDA-approved, so the finished product has not been reviewed for safety, effectiveness or manufacturing quality, and the FDA has said the enforcement discretion that allowed 503A pharmacies to compound semaglutide has ended. It also usually arrives as a vial and a syringe rather than a pre-set pen, which moves dose measurement to you.

  • Tirzepatide vs semaglutide: the head-to-head result, and what the better one costs

They were finally tested against each other. In SURMOUNT-5, 751 adults with obesity took the maximum tolerated dose of one or the other for 72 weeks: tirzepatide lost 20.2% of body weight, semaglutide 13.7%. That gap is real and it is the whole argument for tirzepatide. The other half of the decision is the bill: compounded tirzepatide runs from $85 a month on our board against $69 for semaglutide, and the branded gap is wider still.

  • Wegovy 7.2 mg (Wegovy HD): what the higher dose adds, and what it costs

Wegovy HD is semaglutide at 7.2 mg, three times the standard 2.4 mg dose, in its own pen. In the STEP UP trial it took 18.7% off body weight at 72 weeks against 15.6% for 2.4 mg and 3.9% for placebo, in 1,407 adults with obesity. That puts it within 1.5 points of tirzepatide's head-to-head result and makes it the strongest semaglutide there is. Novo sells it direct for $399 a month, $50 above the standard pen, and it sits above the normal ladder rather than replacing it.
