What to Expect Your First Week on Zepbound | When Does It Actually Start Working?
Our summary
The speaker, who runs the channel The Betty Method, says she lost 110 lb on Zepbound and walks through what people commonly experience in the first week of a GLP-1 medication. She states she is not a healthcare provider, describes herself as a certified personal trainer and nutrition and wellness coach, and says medication, dose and side effect questions belong with a provider. She explains that Zepbound (tirzepatide) acts on both GIP and GLP-1 receptors, while semaglutide acts on GLP-1 alone.

What else the video covers
She says the 2.5 mg starting dose is meant as a loading dose to help the body adjust, not to deliver full results. Reactions vary: some people feel nothing, while others notice mild nausea, sleepiness, fatigue, headache, constipation or diarrhea, usually within the first 24 to 72 hours. She says appetite suppression may be strongest in the first days after the shot and ease later in the week. She describes her own first day, including pushing away a bowl of soup and a sense of contentment that night, and says she was very tired for about two days.
She also covers quieter food noise, a different kind of fullness, and the loss of food as an emotional coping tool, suggesting a journal and an emotional needs menu. She recommends staying hydrated, resting, and eating slowly. She says week-one weight change may be mostly water and does not predict long-term results. She advises against rushing to higher doses or panicking if nothing happens, and suggests picking one habit, such as more protein, rather than overhauling everything. The video ends with promotion of her free guide and a community waitlist.

Questions this video answers
What does the first week on Zepbound feel like?
The speaker says experiences range from nothing at all to mild nausea, sleepiness, fatigue, headache, constipation or diarrhea, usually within 24 to 72 hours. She describes feeling very tired for about two days after her first shot.
Is the 2.5 mg Zepbound dose supposed to cause weight loss?
She says the 2.5 mg starting dose is meant as a loading dose to help the body adjust, not to give maximum results. She adds that some people feel little until week four or after a dose increase.
Does it mean Zepbound isn't working if I feel nothing after my first shot?
She says no. According to the video, the first shot is not a pass or fail test, and responses differ by factors such as insulin resistance, inflammation, metabolism and body weight.
Why does hydration matter in the first week on a GLP-1?
She says people often eat less and so get less water and fewer electrolytes, which can show up as headache, fatigue, nausea and constipation. She calls drinking enough one of the easiest things to try when feeling unwell.

Our guides closest to what this video covers, with prices checked weekly.
- What is Zepbound? Tirzepatide explained: what it treats, how it works, and what it costs
Zepbound is Eli Lilly's brand of tirzepatide, a once-weekly injection that acts on two hormone receptors, GIP and GLP-1. The FDA label approves it, with diet and exercise, for weight reduction in adults with obesity or overweight plus a weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity. In the SURMOUNT-5 trial it produced 20.2% average weight loss at 72 weeks against 13.7% for semaglutide. Lilly sells it self-pay from $299 a month.
- GLP-1 constipation, nausea, diarrhoea and fatigue: the label numbers, and what the labels say to do
The four gut effects are the four most common side effects of every GLP-1, and the labels put numbers on them. On Wegovy 2.4 mg: nausea 44%, diarrhoea 30%, constipation 24%, vomiting 24%, fatigue 11%, against 16%, 16%, 11%, 6% and 5% on placebo. On Zepbound at 15 mg: nausea 28%, diarrhoea 23%, constipation 11%, vomiting 13%, fatigue 7%. They cluster around dose increases, they are the reason 6.8% of Wegovy patients and up to 6.7% of Zepbound patients stopped, and both labels prescribe the same fix: climb slower.
- Stopping a GLP-1: what the withdrawal trials found, and what a maintenance dose actually is
Two trials have measured what happens when you stop. On semaglutide, people who lost 17.3% over 68 weeks regained two-thirds of it in the year after stopping, ending 5.6% below where they started. On tirzepatide, people who lost 20.9% in 36 weeks regained 14% over the next year on placebo while those who continued lost a further 5.5%. Both labels are explicit that these drugs are for maintaining weight reduction long term, which is a different claim from finishing a course. A maintenance dose is simply the dose you stay on; the labels do not define a lower one to step down to at goal.
