Geoff Cook (Noom CEO) - The future of compounded GLP-1s

Patient storyHims House2,096 views54:14Sep 2025

Our summary

Geoff Cook, CEO of Noom, is interviewed on the Hims House podcast about Noom's shift from a behavior change app toward medicated plans. He says that in August more than 50% of Noom's revenue came from its medication plans, up from nearly all revenue being the behavior change program two years ago. He describes Noom's new micro dose GLP-1 program, starting at $119 a month, which uses compounded semaglutide and also includes generic liraglutide. According to Cook, the dose is capped at about 25% of the high maintenance dose, with the aim that 70 plus percent of people have no side effects. He also says he has micro dosed a GLP-1 himself since October 2024, and reports his A1C fell from 5.6 to about 5.2 and he lost 12 pounds.

What else the video covers

The hosts then ask about regulation. Cook says he does not expect a deal that ends compounding in exchange for most favored nation drug pricing, citing the 2013 Drug Quality and Security Act and his view that pharma will not cut cash prices enough. He proposes a "high price drug list" tied to the FDA shortage list as a market-based way to bring down prices. He estimates a 200 to 300% tariff on foreign active ingredients could add roughly $30 per vial, or about 10 to 20% of the price.

Cook also discusses the ended partnership with Eli Lilly, which he says is limited by non-disclosure agreements, and says patients can still get Zepbound and Wegovy through Noom's branded program. He closes with views on competition and customer acquisition costs, a planned free tier, and what he expects from GLP-1s, AI and personalized health platforms over the next 10 years.

Questions this video answers

What is Noom's micro dose GLP-1 program?

Cook says it starts at $119 a month and uses compounded semaglutide, with generic liraglutide also offered. He says the dose is capped at about 25% of the high maintenance dose, and the goal is for 70 plus percent of people to have no side effects. It includes a companion behavior change program, a muscle defense program and an AI body scan.

Why does Noom's CEO take a micro dose of a GLP-1?

Cook says he began in October 2024 because his A1C was 5.6, which he describes as borderline pre-diabetes. He reports his A1C dropped to about 5.2, his LDL improved and he lost 12 pounds, and he says he became more motivated to exercise. He notes this is anecdotal and based on one person.

How would tariffs on foreign ingredients affect compounded GLP-1 prices?

Cook says the active ingredient is a small part of the cost, perhaps $10 to $15. He estimates a 200 to 300% tariff would add about $30, which he puts at roughly 10 to 20% of the overall price, depending on that price.

Does Noom still offer Zepbound and Wegovy after the Eli Lilly partnership ended?

Cook says Noom no longer has an active partnership with Lilly, and that details are limited by non-disclosure agreements. He says patients can still receive Zepbound and Wegovy through Noom's branded telemedicine program, with prescriptions filled through a pharmacy.

Our guides closest to what this video covers, with prices checked weekly.

  • Is Noom legit? What Noom's GLP-1 program costs, its reviews, and its record

Yes. Noom is a real company, and its Noom Med arm prescribes GLP-1s through licensed clinicians. As of October 2026 its own site prices the medication-included Noom GLP-1Rx plan, which is compounded semaglutide, at $129 for the first four weeks, then $279 a month billed as $837 every 12 weeks, though the FAQ on the same page says $249. Trustpilot scores Noom 4.5 across 67,088 reviews, mostly about its app, and we found no FDA warning letter naming it.

  • Microdosing GLP-1s: what it saves you, and what it doesn't

Microdosing a GLP-1 does not save money at a compounded seller: none of the compounded programs we track charges less for a smaller dose. Microdosing is not a medical term, and no dose below the label's starting dose has been studied for this. The one microdosing product on our board costs more a month than full-dose compounded tirzepatide at the cheapest seller we track. Where dose really moves the price is the brands; the provider table, per-mg costs and live numbers are below.

  • Stopping a GLP-1: what the withdrawal trials found, and what a maintenance dose actually is

Two trials have measured what happens when you stop. On semaglutide, people who lost 17.3% over 68 weeks regained two-thirds of it in the year after stopping, ending 5.6% below where they started. On tirzepatide, people who lost 20.9% in 36 weeks regained 14% over the next year on placebo while those who continued lost a further 5.5%. Both labels are explicit that these drugs are for maintaining weight reduction long term, which is a different claim from finishing a course. A maintenance dose is simply the dose you stay on; the labels do not define a lower one to step down to at goal.
