Patient storySimply Sunshine | Weight Loss Journey10,608 views26:00Mar 2024

Our summary

In this check-in, the creator recaps her first full week on Zepbound at 2.5 milligrams. She says she took her first shot at 7:30 on the evening of Tuesday, February 27, after a pharmacy text told her the prescription was ready. She notes that Zepbound is prescribed for weight loss, while Mounjaro is prescribed for diabetes.

What else the video covers

She describes changes she made during the week. She says she is not usually a breakfast eater and found herself forcing food down early on. She disliked the sugar-free creamer she tried, so she experimented with a vanilla protein shake (30 grams of protein, 180 calories) in her coffee, and asks viewers to suggest shakes without an artificial sweetener taste. She names constipation as a side effect she is dealing with, says she started chewable fiber tablets but took too few at first, and plans to take them as directed. She also recommends keeping chewable antacids on hand after feeling unwell following a restaurant dinner with a cocktail and baked haddock.

She then walks through her daily food, including lettuce cups, cantaloupe, protein chips, a zucchini and egg scramble, and a Sunday family dinner where she ate small portions of chicken cordon bleu and mac and cheese without getting sick. She says she stayed between 1,000 and 1,200 calories a day in her tracking app. Her weigh-in on Tuesday morning showed 7.2 lbs lost in the week, and she says she expects some of that to be water weight. She also says her "food noise" dropped substantially. She plans to move her shot to Tuesday mornings.

Our guides closest to what this video covers, with prices checked weekly.

  • Zepbound side effects: the label's table by dose, who actually quits, and how it compares to semaglutide

Zepbound's label puts a number on every common side effect at each of its three maintenance doses. Nausea runs 25 to 29 percent against 8 on placebo, diarrhoea 19 to 23, vomiting 8 to 13, constipation 11 to 17. Across the two approval trials, 4.8 to 6.7 percent of people stopped the drug because of side effects, against 3.4 percent on placebo, and most of those who quit did so in the first months, from gut effects during dose escalation. Below that table are the rarer things the label warns about, with the trial rates where it gives them, and the honest comparison to Ozempic's table.

  • Zepbound dosage chart: every tirzepatide dose, the escalation rule, what each rung lost in the trial, and what it costs

Zepbound has six doses, 2.5 to 15 mg, taken once a week. The label's rule is four weeks at 2.5 mg, then 2.5 mg steps with at least four weeks on each, to a maintenance dose of 5, 10 or 15 mg. In SURMOUNT-1 those three doses lost 15.0, 19.5 and 20.9 percent of body weight over 72 weeks. Direct from Lilly each rung has a price, $299 at 2.5 mg rising to $699 at 10 mg and above, so the chart below carries the dose, the rule, the trial result and the bill in one place, plus the vial and KwikPen concentrations for anyone measuring in units.

  • Mounjaro for weight loss, and why Zepbound exists

Mounjaro is tirzepatide approved for type 2 diabetes. Zepbound is the same molecule from the same maker, approved for weight management, and it goes to the same 15 mg. So unlike Ozempic and Wegovy, there is no dose ceiling pushing you from one to the other: the reason people move is coverage and cash price, not strength. Using Mounjaro for weight is off-label and a prescriber's decision. In the one head-to-head trial, tirzepatide beat semaglutide, 20.2% mean body-weight loss at 72 weeks against 13.7%.
