Mounjaro is a once-a-week injection of tirzepatide. Rybelsus is a once-a-day tablet of semaglutide. Both treat type 2 diabetes. They are different molecules from different companies, and no trial has ever compared them directly.
Before the comparison, one fact changes the question. Novo Nordisk's own support page for Rybelsus now opens with this line:1
RYBELSUS® is being replaced by Ozempic® pill.

So if you are weighing Mounjaro against Rybelsus in 2026, you are really weighing Mounjaro against oral semaglutide, sold more and more under the Ozempic name.

Neither one is approved for weight loss. Lilly's weight brand of tirzepatide is Zepbound, and Novo's weight pill is the Wegovy pill. Using a diabetes brand for weight is off-label: legal, common, and your prescriber's call.
What "being replaced" means for you
NovoCare answers the practical questions on the same page, and the answers are short:1
- Is the new pill different medicine? Novo says the Ozempic pill at 1.5, 4 and 9 mg works like Rybelsus at 3, 7 and 14 mg, with "the same efficacy and safety". The numbers on the bottle change; the drug does not.
- Do you need a new prescription? Yes. Your prescriber has to write the Ozempic pill and pick its strength.
- Does your coverage carry over? Novo says the Ozempic pill "will have the same coverage as RYBELSUS®", with your cost set by your plan.
- Does your savings offer carry over? Yes, you keep using it. No new sign-up.
That matters for this comparison because a choice of Rybelsus today is, in practice, a choice of the Ozempic pill within a refill or two. Everything below about Rybelsus applies to it.
The trials, and why they are not a race
There is no head-to-head trial of tirzepatide against oral semaglutide. What exists is one placebo-controlled trial of each, in adults with type 2 diabetes managed by diet and exercise alone.

The A1C gap looks large. Still, read the table as two separate studies, not a scoreboard. The trials ran for different lengths, enrolled different people, and the weight rows are not even measured the same way: one counts from the starting weight, the other only the loss beyond placebo. A cross-trial comparison is a hint, never a verdict.
In SURPASS-1, nausea ran 12 to 18% on tirzepatide against 6% on placebo, diarrhoea 12 to 14% against 8%.6 PIONEER 1 also found stomach effects the most common, mostly mild and passing.7 Which drug you personally tolerate is not in either table.
The daily pill comes with rules
Rybelsus comes with instructions most pills do not. The maker's own words:3
Take 1 RYBELSUS® tablet by mouth on an empty stomach in the morning with water (no more than 4 ounces). Do not take RYBELSUS® with any other liquids besides water

Then 30 minutes before food, drink or other pills.3 Mounjaro is one injection a week from a single-dose pen.2 Some people would take a daily routine over a needle every time; others would take one shot over seven fasting mornings. Neither preference is wrong, and it is a fair thing to tell your prescriber.
What each costs without insurance

The NovoCare page lists the Ozempic pill at $149, $199 and $299 a month across its three strengths, one bottle of 30 tablets a month.4 At the starting strength, a year of the Ozempic pill from NovoCare costs $1,788, against $5,988 for twelve months of Mounjaro at LillyDirect's $499.
With commercial insurance
Both makers run a card. Lilly's Mounjaro card brings a covered fill to as little as $25, with savings capped at $150 a month. If your plan does not cover Mounjaro, the same card can bring a month to as low as $499.5 Novo's offer for the Ozempic pill is as little as $25 with the saving capped at $100 a month, and it excludes government beneficiaries.4 A Rybelsus savings offer you already hold keeps working on the Ozempic pill.1

The honest summary
On the drug question, tirzepatide has the stronger trial numbers, but nobody has run the trial that would settle Mounjaro against Rybelsus, and the cross-trial gap is a hint, not proof. On the practical question, the pill side of this comparison is now the Ozempic pill, which NovoCare lists from $149 a month against LillyDirect's $499 for Mounjaro, and which needs a new prescription if you are on Rybelsus. On the route, it is a weekly shot against a daily fasting tablet. Take all three to your prescriber; the one you can stay on is the one that works.
