Wegovy / Semaglutide Compound isn't working | Week 34 Wegovy VLOG
Our summary
In this week 34 vlog, Mindy Pedersen says her semaglutide compound is not working as well as Wegovy did. She is on the tenth week of a sublingual rapid dissolve tablet, which she says she bought as a 90-day supply without trying it first. She advises others to buy one month before committing. She crushes the tablet, holds it under her tongue for 5 minutes with water, and then swallows it. She says it tastes bad and leaves sores under her tongue.
One person's experience, in their words. Not medical advice.

What else the video covers
She reports that she lost over 10 lbs in her last month on the 2.4 dose of Wegovy, and less than 5 lbs a month since switching to the compound. She says her hunger was not controlled during a California trip and the following weekend. She also mentions the tablet was out of the fridge for about six days and that she had COVID. She asked the pharmacy to switch her back to the subdermal form, which needs her doctor's authorization because the original prescription was for the rapid dissolve. If that does not help, she says she may look for another compounding pharmacy.
The rest of the video covers her mood. She says her birthday plans were canceled because of COVID, her husband was out of town, and she fell into old eating habits without tracking food. By the end of the week she says she was back on her meal plan and feeling better. Her weigh-in went from 293 to 291.5, which she describes as a 1.2 lb gain. She has a follow-up with her doctor next week to discuss the compound.

- Wegovy 7.2 mg (Wegovy HD): what the higher dose adds, and what it costs
Wegovy HD is semaglutide at 7.2 mg, three times the standard 2.4 mg dose, in its own pen. In the STEP UP trial it took 18.7% off body weight at 72 weeks against 15.6% for 2.4 mg and 3.9% for placebo, in 1,407 adults with obesity. That puts it within 1.5 points of tirzepatide's head-to-head result and makes it the strongest semaglutide there is. Novo sells it direct for $399 a month, $50 above the standard pen, and it sits above the normal ladder rather than replacing it.
- The best GLP-1 for weight loss, ranked by the evidence and then by the bill
By weight lost, the order is settled: tirzepatide (Zepbound) first at 20.2% in the only head-to-head, then semaglutide 7.2 mg (Wegovy HD) at 18.7%, then semaglutide 2.4 mg (Wegovy) at 13.7% to 15.6%, then the pills. By what you pay, the order nearly inverts: compounded semaglutide from $69 a month, the Wegovy tablet from $149, Wegovy pen $349, Zepbound $299 to $699 by dose. The best one is where those two lists meet for your budget and your plan.
- Semaglutide side effects, and what compounding does and does not change
Semaglutide's side effects are gastrointestinal: nausea, vomiting, diarrhoea and constipation, worst early and after each dose increase. Those belong to the molecule, so they are the same whether it came in a Novo Nordisk pen or a compounding pharmacy's vial. What compounding changes is everything around the molecule. A compounded drug is not FDA-approved, so the finished product has not been reviewed for safety, effectiveness or manufacturing quality, and the FDA has said the enforcement discretion that allowed 503A pharmacies to compound semaglutide has ended. It also usually arrives as a vial and a syringe rather than a pre-set pen, which moves dose measurement to you.
