First 12 Weeks on Zepbound 2.5mg Side Effects & Weight Loss Progress
Our summary
Grace, who runs the channel Losing It Grace, shares an update after her first 12 weeks on Zepbound. She says she is 38 and has tried many approaches to weight loss, including Atkins, South Beach, keto, Mediterranean, calorie counting and heavy exercise. She says she lost weight three times before but regained it within a year each time. Recent blood work showed insulin resistance, pre-diabetes, high blood pressure and high cholesterol, and her doctor asked whether she wanted a tool to help. She took her first 2.5 mg injection on February 25, 2025, after a prior authorization was approved.

What else the video covers
She reports starting at 265.4 lbs and weighing 252 lbs on May 8, a loss of a little over 13 lbs in 11 weeks. She says she spent the first eight weeks on 2.5 mg, moved to 5 mg, and is in her third week at that dose. Because of her insurance she filled 5 mg as a 3-month supply, so she plans to stay on it at least 12 weeks before considering 7.5 mg. She also gives starting measurements of 50 inches at the stomach, 60 inches at the hips and 47 inches at the chest.
For side effects, she describes a lot of burping and belching, indigestion when she overeats, constipation and occasional diarrhea. After her 10th injection she developed a rash near the injection site, and the same rash appeared in the same spot on the opposite leg. She says she is seeing an allergist to check for a reaction to an inactive ingredient. She also says she thinks about food far less and exercised at the gym on two recent cruises, which she had never done on earlier cruises.

- How long do Ozempic, Wegovy and Zepbound take to work? The label calendar and the trial timepoints
GLP-1s act from the first dose, but the labels start low on purpose and step up every 4 weeks, so most people do not reach a full dose for 3 to 5 months. Each dose reaches steady levels in the blood after about 4 to 5 weeks. In the trials, people averaged 10.6% weight loss after 20 weeks of Wegovy's escalation and 20.9% after 36 weeks of Zepbound, with the full trial results at 68 to 72 weeks. For type 2 diabetes, Ozempic's A1C results were measured at week 30.
- Mounjaro for weight loss, and why Zepbound exists
Mounjaro is tirzepatide approved for type 2 diabetes. Zepbound is the same molecule from the same maker, approved for weight management, and it goes to the same 15 mg. So unlike Ozempic and Wegovy, there is no dose ceiling pushing you from one to the other: the reason people move is coverage and cash price, not strength. Using Mounjaro for weight is off-label and a prescriber's decision. In the one head-to-head trial, tirzepatide beat semaglutide, 20.2% mean body-weight loss at 72 weeks against 13.7%.
- Zepbound side effects: the label's table by dose, who actually quits, and how it compares to semaglutide
Zepbound's label puts a number on every common side effect at each of its three maintenance doses. Nausea runs 25 to 29 percent against 8 on placebo, diarrhoea 19 to 23, vomiting 8 to 13, constipation 11 to 17. Across the two approval trials, 4.8 to 6.7 percent of people stopped the drug because of side effects, against 3.4 percent on placebo, and most of those who quit did so in the first months, from gut effects during dose escalation. Below that table are the rarer things the label warns about, with the trial rates where it gives them, and the honest comparison to Ozempic's table.
