6 Months of Weight Loss with GLP-1: My Transformation Story
Our summary
Lisa, who runs the channel A Little Less Lisa, reviews her first six months on a GLP-1 medication. She says she took her first shot on March 28th at 183.0 lbs, uses compounded tirzepatide, has never tried semaglutide, and has not gone above 5 mg. She walks through the period in blocks of four weeks, giving pounds lost, the weekly average and total inches lost across her biceps, chest, waist, hips, thighs and calves.

What else the video covers
She reports 15.6 lbs lost in the first four weeks (3.9 lbs per week) but only 6.72 inches, which she attributes mostly to water, inflammation and some muscle. Weeks 5 to 8 brought 8 lbs lost. Her fifth shot was her first jump to 5 mg, and she says she was very sick that week, so she moved back to 2.5 mg and later returned to 5 mg without trouble. Weeks 9 to 12 brought 11 lbs lost, which she links to a trip to Spain where she sometimes ate around 500 calories a day. She says she reached her 40 lb goal on July 11th, then lowered her dose to 2.5 mg, noticed food noise and hunger returning, and went back up to 5 mg.
Lisa describes constipation as her main side effect, managed with magnesium and occasional Miralax, plus red spots at injection sites and nausea that has not recurred for about 10 to 12 weeks. She says she lost a lot of hair and now wears hair pieces, which she calls crowns, and she is trying supplements and laser therapy. Her total is 35.75 inches over about 25 weeks. She says she is not a doctor and does not give medical advice.

Questions this video answers
How much weight did she lose in the first 4 weeks on compounded tirzepatide?
Lisa says she lost 15.6 lbs in the first four weeks, an average of 3.9 lbs per week. She says much of that was likely water and inflammation, and she lost only 6.72 inches in that period.
What side effects did Lisa have on a GLP-1 medication?
She says constipation was her biggest side effect, and she takes magnesium and sometimes Miralax for it. She also reports red spots at injection sites, severe nausea after her first 5 mg dose, and substantial hair loss.
How many inches did Lisa lose in six months on a GLP-1?
Lisa reports losing 35.75 total inches over about 25 weeks. She says this included about 6.5 inches from her hips, 6.5 from her waist and about 2 from each bicep.
Why did Lisa change her dose during treatment?
She says she moved to 5 mg at her fifth shot because hunger was returning, but felt very sick and went back to 2.5 mg. After reaching her goal she lowered to 2.5 mg again, then went back to 5 mg when food noise and hunger returned.

Our guides closest to what this video covers, with prices checked weekly.
- GLP-1 muscle loss: how much of the weight is muscle, and what protects it
On semaglutide or tirzepatide, roughly a quarter to two fifths of the weight you lose is lean mass rather than fat: 25% in tirzepatide's SURMOUNT-1 scan substudy, and 25% to 39% across 20 randomised trials. That share is close to what diet-only weight loss costs. Lean mass is not all muscle, and strength held up in the shorter trials. Resistance training cut the lean share to 17.5% in the pooled trials, and expert groups pair it with enough protein.
- Stopping a GLP-1: what the withdrawal trials found, and what a maintenance dose actually is
Two trials have measured what happens when you stop. On semaglutide, people who lost 17.3% over 68 weeks regained two-thirds of it in the year after stopping, ending 5.6% below where they started. On tirzepatide, people who lost 20.9% in 36 weeks regained 14% over the next year on placebo while those who continued lost a further 5.5%. Both labels are explicit that these drugs are for maintaining weight reduction long term, which is a different claim from finishing a course. A maintenance dose is simply the dose you stay on; the labels do not define a lower one to step down to at goal.
- Retatrutide: up to 28.3% weight loss in trials, no FDA approval yet, and no legal way to buy it today
Retatrutide is Eli Lilly's experimental weekly injection that acts on three hormone receptors (GIP, GLP-1 and glucagon). In the phase 3 TRIUMPH-1 trial, people on the 12 mg dose lost 28.3% of their weight at 80 weeks by Lilly's efficacy measure (25.0% counting everyone randomized). It is not FDA-approved, has no price, and cannot legally be sold. Lilly plans to file with FDA in Q1 2027. Tirzepatide is the closest thing you can buy now, compounded from $85 a month on our board.
