Zepbound - 2 week update on 7.5mg
Our summary
Elise, who posts as For Elise Desiree, says she has moved up to 7.5mg of Zepbound after about six months on the medication. She explains that her first three months of weight loss went well, with 2.5mg in month one and then 5mg for four months. In month four, she says, loss slowed sharply. It took her five weeks to lose the last four pounds toward a 30 pound first goal, and she went two or three weeks with no loss at all despite steps, water, sleep, fiber and protein. She says her doctor agreed that moving up was acceptable even when loss is slower, and she says she felt some shame about the decision after commenters warned against going up too fast.

What else the video covers
She reports side effects since starting 7.5mg two weeks ago. Her appetite suppression has returned, which she says had faded on 5mg. She also says she feels very sleepy and tired, to the point of shortening her workouts from 45 minutes to 30, and that she feels cold much more than before. She notes her labs, including iron and thyroid, were normal a week before the increase, and she is unsure whether the tiredness and the cold are connected.
She also describes a five day trip to New Orleans for her and her husband's birthdays. She says she ate and drank freely with no eating window and was up 5.5 pounds when she weighed in afterward. It took 13 days to get back to 156 pounds. She says she has no regrets, but that she plans to keep a closing time for eating on future trips. She ends by saying she started at 186 pounds and is back at her 30 pound loss.

- Zepbound dosage chart: every tirzepatide dose, the escalation rule, what each rung lost in the trial, and what it costs
Zepbound has six doses, 2.5 to 15 mg, taken once a week. The label's rule is four weeks at 2.5 mg, then 2.5 mg steps with at least four weeks on each, to a maintenance dose of 5, 10 or 15 mg. In SURMOUNT-1 those three doses lost 15.0, 19.5 and 20.9 percent of body weight over 72 weeks. Direct from Lilly each rung has a price, $299 at 2.5 mg rising to $699 at 10 mg and above, so the chart below carries the dose, the rule, the trial result and the bill in one place, plus the vial and KwikPen concentrations for anyone measuring in units.
- How long do Ozempic, Wegovy and Zepbound take to work? The label calendar and the trial timepoints
GLP-1s act from the first dose, but the labels start low on purpose and step up every 4 weeks, so most people do not reach a full dose for 3 to 5 months. Each dose reaches steady levels in the blood after about 4 to 5 weeks. In the trials, people averaged 10.6% weight loss after 20 weeks of Wegovy's escalation and 20.9% after 36 weeks of Zepbound, with the full trial results at 68 to 72 weeks. For type 2 diabetes, Ozempic's A1C results were measured at week 30.
- Mounjaro for weight loss, and why Zepbound exists
Mounjaro is tirzepatide approved for type 2 diabetes. Zepbound is the same molecule from the same maker, approved for weight management, and it goes to the same 15 mg. So unlike Ozempic and Wegovy, there is no dose ceiling pushing you from one to the other: the reason people move is coverage and cash price, not strength. Using Mounjaro for weight is off-label and a prescriber's decision. In the one head-to-head trial, tirzepatide beat semaglutide, 20.2% mean body-weight loss at 72 weeks against 13.7%.
