Do’s and Don’ts for your first week on Ozempic and Mounjaro

Patient storyAfter The Weight208,704 views1:33Mar 2023

Our summary

Melanie, who says she has lost over 70 pounds and has used different weight loss strategies including Ozempic and Mounjaro, shares a short list of do's and don'ts for the first week on these medications. She says not to expect results right away, because everyone starts at the lowest dose. Some people may notice effects early, while others may not see them until they reach a higher dose.

One person's experience, in their words. Not medical advice.

What else the video covers

She says to eat something before the shot and to take it at night, which she believes can reduce side effects as the body adjusts. She also suggests trying the thigh as the first injection site, saying it tends to cause the least nausea. She recommends drinking plenty of water, with a rule of thumb of about half your body weight (in ounces, as she frames it) without going over a gallon a day. She adds that some people see fast early weight loss as the body releases water, though she notes this does not happen to everyone.

For digestive issues, Melanie recommends magnesium glycinate 400 milligrams every night to help with constipation, along with B12, vitamin D and a daily multivitamin. For burps, including sulfur burps, she suggests lower fat foods, smaller meals and Gas-X. She closes by saying some people do not see results for two, three or six months, because the body needs to adjust and the right dose has to be reached.

  • Ozempic side effects, and the ones that actually make people stop

The common Ozempic side effects are gastrointestinal: nausea, vomiting, diarrhoea and constipation. They cluster in the first weeks and after each dose increase, which is why the drug is titrated slowly rather than started at the dose that does the work. Ozempic also carries a boxed warning for thyroid C-cell tumour risk, and the label for Wegovy, the same molecule, advises against combining it with another GLP-1. The practical thing nobody tells you: most people who quit do it during titration, and what decides whether that costs them money is the seller's pause and refund terms, not the sticker price.

  • Mounjaro vs Ozempic, and the trial everyone quotes about it

Mounjaro is tirzepatide and Ozempic is semaglutide: different molecules, not two brands of the same drug. Both are approved for type 2 diabetes, and the weight-loss versions are Zepbound and Wegovy. In the one head-to-head trial, SURMOUNT-5, tirzepatide produced 20.2% mean body-weight loss at 72 weeks against semaglutide's 13.7%. The detail almost every page omits: that trial dosed semaglutide to 2.4 mg in people without diabetes, which is the Wegovy range. Ozempic tops out at 2 mg. The famous result did not test Ozempic's dose range.

  • How long do Ozempic, Wegovy and Zepbound take to work? The label calendar and the trial timepoints

GLP-1s act from the first dose, but the labels start low on purpose and step up every 4 weeks, so most people do not reach a full dose for 3 to 5 months. Each dose reaches steady levels in the blood after about 4 to 5 weeks. In the trials, people averaged 10.6% weight loss after 20 weeks of Wegovy's escalation and 20.9% after 36 weeks of Zepbound, with the full trial results at 68 to 72 weeks. For type 2 diabetes, Ozempic's A1C results were measured at week 30.
