4 Weeks on Tirzepatide vs. My Entire 2025 Weight Loss

Patient storySandy Gibson17,903 views23:36Oct 2026

Our summary

Sandy Gibson, who says she started at 256 lbs, reviews her first four weeks on compounded tirzepatide from the telehealth company Brello. She tracks her numbers closely: she weighs and logs all her food in an app and uses a smart scale for daily weights and body composition. Her Tuesday weigh-in this week was 247 lbs, one pound up from the previous week, which she attributes to water retention from her period. She reports a loss of about nine or ten pounds over the month.

What else the video covers

She says her average intake was 1,484 calories in the first week, with more than 100 grams of protein a day and three meals of roughly 500 calories each. She has not seen a dietitian and says she does not feel it is necessary now. According to the scale app, her body fat went from 55.8% to 53.8% and her skeletal muscle mass from 65.8 to 66.8 lbs, though she notes the figures are not precise and are best used for trends. Her waist is down 1 inch, while bust and hips are unchanged. She describes more energy, better sleep, mild early digestive side effects that have since faded, and lifting and cardio picking up after two light weeks. She also says she changed her breakfast after viewer comments about the fig bars she was eating.

She says she has been on 2.2 mg of tirzepatide, not 2.5 mg, and will move to 4.4 mg for her fifth shot. She compares the month with 2025, when she says she lost 20 pounds over the whole year on a carnivore diet, and she describes the month as a success. She closes by asking viewers about their own first-month results.

Questions this video answers

How much weight did she lose in the first month on compounded tirzepatide?

Sandy says she went from 256 lbs to 247 lbs at her last weigh-in and describes the loss as about 9 or 10 pounds over four weeks. She notes her weight rose one pound that week, which she attributes to her period.

What dose of tirzepatide was she on in month one?

She says she was on 2.2 mg, not 2.5 mg, following the dosing schedule from her telehealth company. She plans to move to 4.4 mg for her fifth shot.

Did body fat and muscle mass change in the first month on tirzepatide?

According to her scale app, Sandy's body fat fell from 55.8% to 53.8% and skeletal muscle mass rose from 65.8 to 66.8 lbs. She says the numbers are not exact and are most useful for trends.

Our guides closest to what this video covers, with prices checked weekly.

  • GLP-1 muscle loss: how much of the weight is muscle, and what protects it

On semaglutide or tirzepatide, roughly a quarter to two fifths of the weight you lose is lean mass rather than fat: 25% in tirzepatide's SURMOUNT-1 scan substudy, and 25% to 39% across 20 randomised trials. That share is close to what diet-only weight loss costs. Lean mass is not all muscle, and strength held up in the shorter trials. Resistance training cut the lean share to 17.5% in the pooled trials, and expert groups pair it with enough protein.

  • Retatrutide: up to 28.3% weight loss in trials, no FDA approval yet, and no legal way to buy it today

Retatrutide is Eli Lilly's experimental weekly injection that acts on three hormone receptors (GIP, GLP-1 and glucagon). In the phase 3 TRIUMPH-1 trial, people on the 12 mg dose lost 28.3% of their weight at 80 weeks by Lilly's efficacy measure (25.0% counting everyone randomized). It is not FDA-approved, has no price, and cannot legally be sold. Lilly plans to file with FDA in Q1 2027. Tirzepatide is the closest thing you can buy now, compounded from $85 a month on our board.

  • Mounjaro for weight loss, and why Zepbound exists

Mounjaro is tirzepatide approved for type 2 diabetes. Zepbound is the same molecule from the same maker, approved for weight management, and it goes to the same 15 mg. So unlike Ozempic and Wegovy, there is no dose ceiling pushing you from one to the other: the reason people move is coverage and cash price, not strength. Using Mounjaro for weight is off-label and a prescriber's decision. In the one head-to-head trial, tirzepatide beat semaglutide, 20.2% mean body-weight loss at 72 weeks against 13.7%.
