The number, and what changed in how we count it

The GLP-1 Price Index reads $149 a month on 2 October 2026: the median all-in price of the cheapest verified offer at each of the 22 programs we list, membership fees included. In August it opened at $158 across 35 programs.

Most of that difference is not the market. Since October we list only programs we can send readers to through a working partnership, so 13 programs left the index. On the same 22 programs, the median was $149 on 23 August, $149 on 13 September and $149 on 2 October. Like for like, the price of a GLP-1 program has not moved.

Underneath the headline, GLP-1 telehealth prices did not move in six weeks: the same 22 programs had a median of $149 a month on 23 August and on 2 October. What moved is how much you pay up front to get the advertised number. Ten of 22 programs only reach their lowest price on a multi-month plan.

The advertised price usually needs a year up front

Ten of the 22 programs reach their lowest price only on a multi-month plan. The clearest case is the floor itself. Embody's $69 semaglutide is $69 a month on a 12-month plan, $828 paid before the first dose; month to month it is $79. SnagRx quotes the same $69 with no term stated before its intake quiz.

That changes the answer for anyone who expects to stop. Over six months, Embody's cheapest route is its 6-month plan at $438, not the 12-month plan, which still bills the full year. Our cost calculator now does this sum for every program.

The moves were at the edges

Two programs repriced their entry offer. Gala Health rose from $149 to $179 on 13 September, when its microdosing page disappeared and its cheapest standing plan became the $179 yearly one. Pallas Health went the other way: $199 on 13 September, back to $139 on 2 October as an annual-plan average, the same figure it showed in August.

Neither moved the median, because the middle of the market held still.

One molecule, still 4.3 times apart

Compounded semaglutide sells for $69 to $299 a month across the 13 programs that list it: the same 4.3-fold spread we measured in August. Compounded tirzepatide runs from $119 to $399 across 13 programs. Same molecule, same day, different websites.

Insurance moved on 1 October

CVS Caremark said Zepbound would join Wegovy as a preferred weight-loss GLP-1 on its commercial formularies on 1 October 2026, and Aetna listed it for addition to its Standard Control formulary on that date.
